Complete one of these forms for each grade+subject or course (e.g., 5 Grade Science) requiring assessments.

Pacing

 Write “P” if the test assesses previous grade/level standards.  Write “J” if the test assesses material just taught (e.g. in line with a pacing guide).  Write “C” if the test assesses comprehensive material (standards taught since school started, and not just because this is the first assessment in a series).

 Describe the source from which each test will come (e.g., Create, Revise District Benchmark, etc.).

tests, some columns will be left blank)  Write the # of questions/items on each test in the series and the test’s nature: M = multiple choice questions O = open response questions T = tasks the teacher scores W = written composition Other = describe  Write “E” if the test focuses on essential standards or aligns with weighting in the blueprints (including if the test series as a whole aligns with blueprint weighting).  Write “A” if the test focuses on all standards somewhat evenly.  Write “SS” and/or “CC” if the test focuses on state standards vs. national common core.  Write “S” if the test focuses on skills not necessarily tied to specific standards (e.g. phonics).
