The C.A.R.E. Award is designed to recognize associates for performance that goes above and beyond that which is required in the course of normal job responsibilities. It also serves as a motivation to associates, to urge them to be a part of the outstanding H. D. Smith team, and continue to strive to do more than “just what is required.”

The Award

C.A.R.E. Award recipients receive a Certificate of Appreciation (suitable for framing), a $100 non-profit donation, $100 cash award and day off with pay!

Selection Criteria

Award recipients will be selected based on performance, actions or services related to the betterment of H. D. Smith, its customers and associates, as well as, the associate’s demonstration of understanding the mission and values of H. D. Smith. Performance, actions or services required as part of the associate’s regular job responsibilities are not eligible for recognition. Certain behaviors such as being kind to customers on a daily basis; assisting when shorthanded; demonstrating hospitality to visiting customers, guests and auditors; and smiling and being kind are expected of every H. D. Smith associate. Any associate may nominate a fellow associate by properly completing and signing a nomination form, available on myhdsmith.com. Submit the properly completed and signed form to your manager, Human Resources Representative or Vice President Division Manager (VPDM) at your facility. These forms must be specific and descriptive of the action that prompted the nomination. Anonymous forms will not be considered and will be discarded.

Nomination ProcesS

The manager or VPDM will review the nominations for approval and submit to Corporate Human Resources for final approval and review of the nominations for adherence to selection criteria and proper completion of submission form. Please reference the following criteria as it relates to the nominee: Customer Focused • Consistently serves as a role model for the mission and values of our Company • Collaborates with other departments in support of quality customer care • Demonstrates through quality work the company’s desire to support quality patient care Results Driven • Often goes above and beyond normal job duties • Has proved to be an invaluable member of the department team • Shows strong initiative in performing job duties without constant management guidance • Upholds the highest standards of excellence in work

Action Oriented • Has the ability to resolve difficult situations with positive outcomes • Recognizes opportunities to improve and takes initiative to implement • Motivates others and self to execute excellent work Exceptional Service • Takes internal and external services to the next level by anticipating and exceeding customer needs • Receives positive feedback from customers and colleagues • Frequently performs tasks that go above and beyond customer or colleague’s normal expectations
