CONSENT FORM FOR PARENT(S) TO READ AND SIGN Dear Parent, It’s an honor for us to lead your son or daughter on such a tremendous mission experience. Thank you for parenting them in such a way that they value this kind of outreach opportunity, and thank you for allowing them to join us on this journey! We’re looking forward to leading them to be part of what God is doing. Please read the information provided below and sign as your consent. Details of Project: Dates are July 22-28. We will be leaving Sunday morning and will be back the following Saturday evening. Cost of this trip is $250. This covers all travel, food and the cost of the trip. Payment Due Dates: February 11th- $20 Due (Along with application) •

April 15th- $100 Due May 30th- $130 Due We will be partnering with Big Creek Missions to do some ministry and work projects in the area. We will also have morning and evening sessions of worship and reflection to prepare us for the day and then help us also debrief the day. It’s gonna be an awesome week! As the parent of this student, we want to make sure that you know what this trip is about and feel comfortable with your student joining us in this way.
