Combined Youth Event: MAIN EVENT Who: WHCC Youth Ministry (Middle & High School) When: Friday, April 12, 7:00-10:30 p.m. Where: Main Event Stafford, 12626 Fountain Lake Circle, Stafford, TX 77477 What: Game night at Main Event with other churches in Houston! Cost: $25 includes: · Dinner (two slices of pizza and drink) · $10 arcade card · Unlimited bowling, gravity ropes, laser tag, shuffleboard and billiards Payment can be made with cash or checks made out to “Fort Bend Community Church.” Transportation: Option 1: Parents/Youth will provide their own transportation to Main Event. Drop off at 7:00pm, pick up at 10:30pm Option 2: Parents may drop off youth at WHCC at 6:20pm and be shuttled to Main Event. Parents must pick up youth at WHCC at 10:30pm. PERMISSION SLIP & LIABILITY RELEASE

I, ____________________________ (name of parent who has a teenager under 19 years of age), give my teenager _______________________________ permission to attend the above church sponsored youth outing. I release Katy Christian Community Church, its pastors, ministers, elders, deacons, governing council, and volunteer youth staff from any liability arising out of any injury, damage, or loss that may be sustained by the aforementioned minor in connection with travel to, attendance at or participation in the above youth outing. In the event of an accident or injury, I give the youth staff of Katy Christian Community Church permission to secure necessary medical treatment deemed necessary by medical personnel and will not hold the church liable for such treatment. I also agree to bear all incurred medical expenses.
