APPLICATION FOR BOOTH SPACE—Please fill the application out COMPLETELY or the application will not be processed and will be returned. Booths will not be assigned until signed application and signed rules, arts & crafts booth or food trailer photographs, and payment are received.

Arts and Crafts (A&C) Vendors are REQUIRED to SEND PHOTO of exhibited items in booth. Photos must be sent EACH YEAR. If you wish to have them sent back, please send a SASE and they will be returned. Food and A&C Vendors: Provide detailed description of booth/items (continue on back if more space is needed). Include space requirements and serving side of trailer. This section must be filled out even though photos are sent. If this section is not filled out, we will be UNABLE to place you in the show.

Food Booths: All food vendors must present a State of Texas Health Permit and a photo of the food trailer. Please indicate customer openings as your trailer faces the Courthouse. Left _______, Right_______, one end or two_______. A&C Booths: All A &C booths are required to have 70% homemade A&C product. If you do not meet these requirements, please choose an R & R Booth. R&R Booths: R& R booths will consist of any retail or resale items. There are no homemade requirements. Please check booth requirements above then indicate your 1st, 2nd and 3rd choice of booth below. Please note, Booths marked “F” are food booths, and those marked “S” are Super Booths (10x20). A map is provided for your use. Please do not select the same number you had last year as the maps have been revised, check booth location and number before indicating your choices. A&C Booth , Food Booth , R& R Booth _______CHOICE—1st , 2nd , 3rd With Electricity $20.00 per 20 AMPS plug-in; please note – NO GENERATORS ALLOWED. With Water $10.00 per water plug-in *You must list what appliances and amount of amperage needed if applying for electricity. You are required to supply

your own

extension cord of at least 100 feet or longer.

Booth Price + Electricity + Water = Total Amount Enclosed: Entry deadline is November 9, 2017 when paying with check, after November 9, 2017 cash, credit card or money order only. Cancellations after October 19, 2017 will not be due a refund. Submission of an application does not guarantee acceptance. Crockett Area Chamber of Commerce officials reserve the right to select vendors. I hereby give permission for photos of my booth, including merchandise, staff, and/or customers to be used by CIC for festival promotion. I, by my signature below, do indemnify and forever hold the Christmas in Crockett Festival, its officers, directors, agents and members, City of Crockett, or Houston County blameless from any failure to comply with the reporting of the taxes, nor any liability for any accident or any other problems we cannot control. I have read and will abide by the conditions set forth in the vendor rules and this contract.
