Congratulations on completing a Drive 4 UR School event! We hope that your experience was an exciting and rewarding one. In order to fulfill your donation in a timely manner, we ask that you print, complete and return this check request form with your waivers. It is recommended that you return the forms via a reliable shipping service such as FedEx or UPS, and obtain a tracking number for your records. Please remember that we cannot cut checks to the dealership or an individual. Also, checks cannot be sent to a P.O. Box and will only be issued to one school. However, we are happy to have the check delivered to either the dealership or the school at your discretion. Please allow 6-8 weeks for delivery. Thank you for your participation!
