EOHHS is the largest secretariat and oversees 15 operating agencies with combined annual budgets of over $15 billion. For purposes of this RFQ, EOHHS shall mean the Executive Office of Health and Human Services and the agencies it oversees. Please find below a detailed list of hardware specifications. Responses to this RFQ are limited to ITC47 category 2 and ITC47 category 6 vendors that are authorized to sell OEM products. EOHHS reserves the right to negotiate with the awarded hardware vendor for additional units within the next 12 months provided the additional units are quoted at the same or better terms as the initial equipment received pursuant to this RFQ. All vendors that meet the eligibility requirements identified above must submit the following information by the date and time identified in the Timeline section of this RFQ: 1. The name, mailing address, telephone number, and e-mail address of the person authorized to negotiate any resulting order or address any questions regarding the bidder’s response; 2. Identify the appropriate Commonwealth of Massachusetts Statewide Master Agreement that makes you eligible to respond to this RFQ (ITC47 category 2 or ITC47 category 6 vendors that are authorized to sell OEM products); 3. For OEMs only. Please confirm that you will work with any selected deployment vendor that EOHHS selects via the ITC47 Category 6 contract; 4. All eligible bidders can only propose two solutions per each configuration noted below. 5. For Non-OEMs that are authorized to sell OEM products via ITC47: If an OEM is responding to this RFQ directly you may not propose a configuration that differs from the configuration that is being proposed by the OEM. Also, if EOHHS selects a different OEM that will not affect your ability to respond to the deployment RFQ; 6. Identify any minimum amounts of each configuration that must be ordered to prevent an increase in the per/unit cost proposed in your response; 7. Identify the computer hardware specifications of each proposed solution; 8. Total cost of each solution based on the quantities and configurations identified below; 9. Please submit an anticipated shipment schedule that will permit the first site beginning on or about August 30, 2013 and future sites following the pattern identified in the notes below and all systems delivered by September 16, 2013; and 10. All bidders must identify the turnaround time from the date of acceptance of the quote to deliver 2 systems of configuration #1 so that image and testing work can begin immediately and ship 2 units for immediate delivery Configuration #2 for imaging and testing.

Notes: 1. The chosen hardware vendor will coordinate delivery with a services vendor chosen by EOHHS. The services vendor will oversee deployment by taking receipt and imaging all units at their site prior to their delivering to designated sites. Once a vendor has been selected, hardware selections may vary due to the need of the individual locations. EOHHS will submit the accepted hardware and services vendor’s quotes to ITC49 approved lease vendors for purchasing and a 4 year lease cost.
