ENGLAND BASKETBALL AFFILIATIONS 2007-08 BUSA have again been working with England Basketball to provide university clubs with the most cost effective England Basketball Affiliation scheme available. The affiliation scheme provides university clubs and its members with many great benefits that are only available to affiliated clubs. Enclosed is the following information to make the process as easy as possible; • Instructions on how to complete the England Basketball affiliation form • England Basketball Affiliation Form (separate attachment) • List of benefits available to all clubs and its members • List of important England Basketball and BUSA contact details (use these if you are having difficulty appointing officials) • Premier League referee appointment requirements (separate attachment) Universities are asked to complete and return the affiliation form (electronic copies only) to [email protected] no later than Friday 2 November 2007. This is a compulsory affiliation scheme for all English institutions wanting to compete in BUSA Leagues 200708. Teams that do not complete payment and the attached paperwork will concede a walkover for every match following Friday 2 November 2007 up until the date where by an institution has registered a team(s). Walkovers will not be conceded for any matches that have been completed prior to this date. Should you have any queries then please do not hesitate to contact me. How to complete the England Basketball Affiliation Form Each university MUST complete the attached excel form, pay the correct fee and return it to the BUSA office in order to receive their individual membership cards. Therefore each university is required to complete the following steps in order to fully affiliate to England Basketball. 1. Complete the attached form ELECTRONICALLY and E-MAIL to [email protected] 2. Complete all players details as thoroughly as possible 3. If you are affiliating any coaches put their names in the bottom row of the affiliation form and place a “(c)” after their last name 4. Teams can affiliate a maximum of 14 players/coaches as part of the initial team fee. Fees are £130.00 for your first team and £110.00 for any team after. If you are affiliating both a Men’s and Women’s team they are regarded as both 1st teams and will cost £130.00 each to affiliate. 5. Teams wishing to register more players throughout the season must complete the affiliation form and pay £8 per player. This applies once you need to register more than 14 players/coaches. 6. Players and Coaches who are registered through either National League or their local league do not need to re-affiliate through BUSA. These players must be able to produce a valid England Basketball membership at the request of any EB/BUSA official at any BUSA fixture. 7. Ensure the voting representative has inserted an electronic copy of their signature 8. Pay the correct amount owed to BUSA. Payment can be made by BACs or cheque (payable to BUSA) 9. Once full payment and the completed electronic form have been received BUSA will issue membership cards to each university. 10. Once universities have received the memberships cards, they are required to do the following; a. Membership card numbers correspond to the number on the returned affiliation form. This can be found in the right hand column of the affiliation form. Ensure that the corresponding name is written onto the correct card. b. Fill out the memberships cards with the name of the player and other details required. c. Attach a passport sized photo and laminate ALL FORMS MUST BE COMPLETED AND RETURNED (ELECTRONIC COPIES ONLY) TO [email protected] BY FRIDAY 2 NOVEMBER 2007. FORMS WILL NOT BE ACCEPTED BY POST OR FAX. FAQ’s What if my team is National League and has already affiliated? England Basketball has included a new tickbox on National Leagues forms to indicate if National League players are playing both National League and BUSA competitions. Please tick this box and BUSA will be notified automatically by England Basketball that these players are affiliated. These players must be able to produce a valid England Basketball membership card at the request of any EB/BUSA official at any BUSA fixture.

What if my team has affiliated through our local league or England Basketball directly? If teams have already registered they will need to provide BUSA with written notification before Friday 2 November 2007. Email a copy of the completed England Basketball Affiliation form to [email protected]. These players must be able to produce a valid England Basketball membership card at the request of any EB/BUSA official at any BUSA fixture. We are a university based in Wales/Scotland, who do we need to contact to affiliate? Scottish Universities should contact Stew Fowlie, Sports Manager at Scottish Universities Sport (SUS) on [email protected] or 0131 650 9653. SUS are running the registration scheme on behalf of Basketball Scotland. Welsh Universities can find registration information on www.basketballwales.com or contact Pat Sanders, Basketball Wales Registration Secretary on [email protected] or 07733 270 567. If teams have registered through Basketball Wales or Basketball Scotland they will need to provide BUSA with written notification before Friday 2 November 2007. Email a copy of the completed Affiliation form to [email protected]. These players must be able to produce a valid Governing Body membership card at the request of any BUSA/game official at any BUSA fixture. What if I have any further questions?
