INTRODUCTION This document defines the requirements, deliverables, tasks, and scope of the work to be delivered by a qualified VENDOR to the DEPARTMENT OF PUBLIC HEALTH (“DPH”). VENDOR shall provide the services necessary to accomplish all of the tasks and subtasks in this Statement of Work (SOW) and to successfully install, configure, test, and implement all of the solution components including, but not limited to, all associated software and/or firmware, patches/fixes, and interfaces, post implementation support, technical and end‐user training. VENDOR is responsible for all acts, and services required to provide DPH with a fully functional System in accordance with the Manufacturer’s Specifications, whether or not the acts or services required to do so are specifically identified as a task, subtask, or deliverable in this Statement of Work. DPH is responsible for procuring and taking delivery of all required solution components prior to commencement of installation and engineering services. Vendor Qualifications All configuration and integration work must be completed by VENDOR with the following credentials. VENDOR must be an authorized EMC VELOCITY PREMIER/SIGNATURE LEVEL PARTNER & EMC VSI (VELOCITY SERVICES IMPLEMENT) partner with EMC certified implementation engineers on staff. In addition, VENDOR must have VMware Enterprise level certifications with Infrastructure Virtualization specialization. In the event technical support or other services are required during or after the end of the project due to problems resulting from this project, one of VENDOR’s certified engineers must be able to respond on site within four hours for diagnosis and problem resolution or escalation. All work described in this Statement of Work shall be performed by VENDOR, no subcontracting. All equipment must be supplied in brand new, unused, condition. All equipment must come from the manufacturer or an authorized EMC Reseller with full USA manufacturer warranty and USA manufacturer technical support. Equipment shall not be gray market. Scope Details         

Meets with customer to ensure that the environment and operational implementation requirements (hardware, software and infrastructure) are met by the customer. Plans and estimates schedule for the installation and configuration tasks for services. Verifies the installation and configuration results. Rack and Stack of up to 5 Disk Array Enclosures (DAE). Verify bus and enclosure address for DAE(s). Installation of (41)SAS Drives on DAE-6 Enclosure. Installation of (41)SSD Drvies on DAE-5s Enlcosure. Verify Enclosure / Drives in Unisphere GUI. Configure up to two (2) RAID Groups or Storage Pools as per customer storage requirement.

   

Configure up to six (6) LUNs and two (2) servers / hosts. Format and verify storage. SAN basic functional overview / knowledge transfer. Provide a design workbook that lays out the drive configuration.

DPH Responsibilities    

 

Provide at least one technical contact with system administration responsibilities and appropriate system/information access privileges. Ensure that all environment and operational requirements are met prior to commencement of the services. Verify that the equipment location(s) is prepared prior to the commencement of the services. Provide VENDOR onsite/offsite personnel and authorized agents with access to the DPH’s systems and networks (including, without limitation, remote systems and remote network access) as necessary to perform the services during normal business hours, or at mutually agreed times. Provide support from technical support teams for all vendors and third parties, as necessary. Responsible for network connectivity, performance, and configuration issues.

Services Scope Changes Any changes to the services or the schedule must be mutually agreed upon by VENDOR and the DPH in writing. Depending on the scope of such changes, a separate Professional Services Statement of Work detailing the changes, the impact of the proposed change on the charges and schedule, and other relevant terms, must be mutually agreed to in writing. Additional Information Location of installation and configuration: 5555 Ferguson Avenue, Los Angeles, CA Hours of installation: TBD. Site preparation can be completed 8-5 Monday-Friday with scheduling approval from DPH staff. Any disruptive work that affects access to the attached production servers will be scheduled for weekend hours. Expected Completion Date: On or before January 2015.
