ENDURING ENDEAVORS ESTATE, LLC GUEST WAIVER AND RELEASE FORM Enduring Endeavors Estate, LLC ("EEE") has agreed to provide to _______________________________ (the "Guest(s)"), access to and use of approximately 74 acres of land at 1724 County Road 100 N, Washburn, Illinois, 61570, including a house, barn, other outbuildings, timberland, a pond, etc. (the "Premises"). The Guest(s) has/have no business or organizational affiliation with EEE and will use the Premises for purposes and activities unrelated to business conducted by EEE. 1.

The Guest(s) acknowledge(s) that EEE makes no representations, warranties, or assurances regarding use of or the safety, condition, or nature of the Premises and assumes no liability with respect to use of the Premises. The Guest(s) hereby voluntarily accept(s) and assume(s) any and all risks and dangers associated with use of the Premises.

In consideration for the right to access and use the Premises, the Guest(s) hereby waive(s), release(s), and forever discharge(s) Enduring Endeavors Estate, LLC, its affiliates, officers, agents, representatives, and all others from any and all responsibility or liability for injuries or damage resulting from use of the Premises or for the loss, theft or damage of any article of personal property which may occur during use of the Premises. The Guest(s) also hereby release(s) all of those mentioned and any others acting upon their behalf from any responsibility or liability for any injury, loss or damage to individuals participating in use of the Premises, including those caused by negligent act or omission of any of those mentioned or others acting on their behalf or in any way arising out of or connected with participation in use of the Premises.

The Guest(s) agree(s) to indemnify and hold harmless EEE from any loss, cost, damage, expense or liability by reason of personal injury or property damage, including attorney fees and costs, asserted by any third party as a result of the use of the Premises.

The Guest(s) has/have read and fully understand and agree to this Use of Premises Waiver and Release Form and expressly release, discharge and hold harmless EEE from any and all claims and damages.

5.

The Guest(s) understand(s) that signature on this form operates as a release of any claims as stated above, and freely and willingly consent(s) to this Guest Waiver and Release Form.
