Global Convenience

Global Convenience The U.S. Bank Executive Card contains both EMV Chip technology and a traditional magnetic stripe. Widely used outside the United States, EMV Chip cards are embedded with a microchip and use either a signature or a Personal Identification Number (PIN) for transaction verification. Combining both technologies on your card ensures that your purchases can be conveniently processed around the world. Using your EMV Chip card When using a chip-reading terminal, follow this process: • Insert your card into the terminal • Leave your card in the terminal for the duration of the transaction • Follow the prompts on the terminal as a guide, and either sign your name or enter your PIN • Remove the card once the transaction is complete

Program Benefits Automatic $1 Million Travel Accident Insurance Coverage Executive cardholders receive premium coverage automatically so they can travel with confidence. This supplements any other insurance policy which is provided for clients individually. • The Executive Card offers $1 million Travel Accident Insurance when traveling worldwide for business purposes on any common carrier (including taxi, bus, train or airport limousine). • Insurance is provided automatically when the entire travel fare is charged to the Executive Card. It is not necessary to notify U.S. Bank when tickets are purchased.

Concierge Service1 Executive cardholders receive complimentary concierge service to assist with all entertainment, business and travel plans. With just one phone call, executives can receive assistance on a number of services including: • Restaurants • Health Clubs • Hotels • Sporting events • Sight seeing tours • A irline travel • Golf tee times

10% Discount on Carey Worldwide Chauffeured Limousine Services The U.S. Bank Executive Card entitles executive cardholders to a 10% automatic discount on any limousine service rental with Carey Limousine. Carey provides complete chauffeur driven limousines, sedans, vans and minibuses in 550 cities and 60 countries.

• Exclusive toll‑free reservations line for executive cardholders at 800‑336‑4646 • Cardholders simply specify the transaction pertains to Account Number 016305, the U.S. Bank Executive Card program • Reservations must be made at least 12 hours in advance for U.S. and Canada, 48 hours in advance for overseas locations • Please give notification of any cancellation within 12 hours for U.S. and Canada and within 24 hours for overseas locations • May not be combined with any other offer or discount

Member in Avis Preferred Service Program Executive cardholders can take advantage of the Avis Preferred Service Program by calling 800‑525‑7530 to enroll. As a Preferred Service member, executives enjoy speed renting services at more than 250 locations across North America and hundreds more around the world. • Reservations can be made by calling the exclusive Avis Preferred Service line at 800‑331‑7423 • Executive cardholders receive a free upgrade on their first rental at most Avis preferred service locations • Executives stop at the special Preferred Service counter and show their driver’s license • A n Avis Representative will have the already processed rental agreement and keys to their car waiting just outside the airport terminal door

Additional services include “as‑directed” hourly services, airport transfers and group programs. English speaking chauffeurs and chauffeur guides are available throughout the world.

At airports where the Avis facility is away from the main terminal: • The Avis courtesy bus takes executives directly to their waiting car with rental agreement and keys inside • Executives simply show driver’s license and rental agreement to the parking attendant and proceed to their waiting car

Complimentary Upgrades From Golden Tickets, Inc.* Executive cardholders can order tickets for any sporting or entertainment event through Golden Tickets and receive an automatic upgrade to the next best available seating. Golden Tickets provides ticket brokerage services for events across the country. Reservations can be made by calling the toll‑free number at 800‑288‑2461 at least three weeks in advance. By specifying that the transaction will be placed on a U.S. Bank Executive Card, executives receive: • Automatic upgrade to the next best ticket category • Best quality seating available charged for the price of the next less expensive level

Concierge Questions & Answers How do I get these services? Have your account number ready and call the toll-free MasterCard Concierge Service phone number at 800‑336‑6420. You can also access our website at http://concierge.mastercard.com. How does the concierge know I am eligible for this service? The concierge will identify you by your 16‑digit MasterCard account number. How long will my request take? Depending on the request, the concierge may be able to assist you on the phone. If the request requires additional research, then the concierge will email or place a follow‑up call to you. Most requests are completed within 24 hours. Your concierge will notify you of the anticipated time for completion. How do I pay for what the concierge has purchased? The concierge will advise you of the costs. The items or services purchased will be billed to your Executive Card. There is no cost for the concierge’s time or assistance.

When is this service available? The service is available 24 hours a day, seven days a week. Is there a limit on the number of times I can use this service? No, you may use the service as often as needed. What types of concierge services are available? Concierge services are available for entertainment planning, business services, travel information and assistance, gift arrangements, country or city information, pre‑trip assistance and specialty services. Why should I try the Concierge Service? MasterCard Concierge Service has a wealth of knowledge and resources and has completed thousands of requests for busy travelers throughout the world. Questions & Answers U.S. Bank answers commonly asked questions about the benefits of the Executive Card. Cardholders may call U.S. Bank at 800‑344‑5696 for answers to questions not listed below. How do I receive my Executive Card benefits? • $1 million Travel Accident Insurance — Executive cardholders are automatically covered with their U.S. Bank Executive Card. • Golden Ticket Upgrades —  When ordering tickets through Golden Tickets at 800‑288‑2461, Executive cardholders simply indicate they are a U.S. Bank Executive cardholder and would like the transaction placed on their U.S. Bank Executive card. Cardholders will be automatically upgraded to the next best ticket category. Ticket request should be made at least three weeks in advance and are subject to availability. • Carey Worldwide Chauffeured Limousine Services —  Executive cardholders should call 800‑336‑4646 and identify themselves as a U.S. Bank Executive Cardholder, account #016305 to receive a 10% discount on reservations. Services must be paid for with the U.S. Bank Executive Card. • Avis Preferred Service —  Call 800‑525‑7530 to enroll and receive additional information regarding membership.

What is my insurance coverage as a U.S. Bank Executive Cardholder? • $1 million coverage Travel Accident Insurance —  automatic insurance against accidental bodily injuries that are the sole cause of death or dismemberment while riding in, boarding or alighting from any aircraft or land or water conveyance operated by a common carrier licensed to carry passengers for hire, provided the entire travel fare(s) has been charged to the U.S. Bank Executive Card.

Questions & Answers

• Auto Rental Collision Damage Waiver — Provides coverage for damage due to collision or theft of most vehicles. Coverage is primary when used for business purposes for eligible rentals up to 31 consecutive days. Certain restrictions, limitations and exclusions apply. Refer to the MasterCard Commercial Guide to Benefits brochure for details. • Lost Luggage Reimbursement —  When paying for the cost of common carrier ticket with the U.S. Bank Executive Card, executive cardholders are eligible to receive excess reimbursement for the cost of replacement luggage and its contents up to the amount paid to the cardholder by the common carrier, or $5,000 — provided the luggage was checked with the common carrier and lost due to theft or misdirection by the carrier.
