Below is a quick reference for YTP Exit and VR Closure Processes. YTP Exit Decision Process   



The Exit Decision is made by the team, including the young adult, TS, and Vocational Rehabilitation Counselor. Consider Exit from YTP when the young adult no longer requires support from the YTP Transition Specialist. The date of the Exit Decision begins 12 months of follow-up to track engagement. Data at 6 mo. and 12 mo. are only entered in database after EXIT date is added. Once 12 months of follow-up has been recorded in the YTP Database, the participant has reached YTP Completion. Engagement means employment or enrolled in post-secondary education or training, or a combination of employment and education/training, consistent with a participant’s abilities and interests and when applicable, the student’s IPE.

Vocational Rehabilitation File Closure Process 

 

Two closure statuses: Rehabilitation or Other can be a unilateral decision made by the VRC. The student and TS should be informed of the decision. Ideally, the TS is informed prior to the decision, as a TS can help move the participant from a status of Other to Rehabilitation. Closure Status as Other range from “non-cooperation” to “moved and unable to locate”. Closure Status as Rehabilitation means employment is o Integrated with competitive wage o Consistent with Individual Plan for Employment (IPE) o Sustained for at least 90 days, and the participant and VRC consider the employment outcome satisfactory, and stable if receiving supported employment.
