Grace Chapel Student Ministry Scholarship Application
A scholarship fund has been established with money accumulated through fundraising opportunities and efforts by the Student Ministry at Grace Chapel. This fund is administered by the Youth Pastor and the members of the Youth Staff at Grace Chapel. We want every student to have the opportunity to be involved in the events put on or attended by Grace Chapel Student Ministries. Lack of funds should not be a determining factor for choosing not to attend. Every year we have limited scholarship funds available for our ministry opportunities. Upon completion of this application and review by the adult members of our Youth Staff, scholarships will be awarded on the basis of need and current level of involvement of the applicant within the Student Ministry at Grace Chapel. Who is eligible to receive a scholarship? -‐ Any student who is a member of Grace Chapel -‐ Any students who needs financial assistance for the event(s) -‐ Any student who is eligible to participate in the event(s) -‐ Any student who has been an active participant at Grace Chapel Student Ministry for at least 6 months as determined by the Grace Chapel Youth Staff What events are these scholarships for? -‐ Momentum Youth Conference (Grades 9-‐12) -‐ Beyond Middle School Camp (Graces 6-‐8) Is there a deadline for the scholarship application? Yes! The application must be submitted to the Youth Pastor one week prior to the close of early registration for the event(s). After the application is submitted the Youth Pastor and Youth Staff will review and respond prior to the close of early registration. In the event that a scholarship is issued and the student(s) do not attend the event(s), it is the responsibility of the recipient and his or her guardian(s) to repay the full scholarship amount to Grace Chapel Student Ministry. The only exception to this will be if a refund is granted by the event(s) once the scholarship funds have been paid. For Which Event Would This Scholarship Be Used? Momentum or Beyond Is the Applicant a Regular Attender at Grace Chapel Student Ministry? Y or N Why Should You Be Considered for a Scholarship? Why Do You Think Attending This Event Will Be Beneficial For You? Along with this application you will need 2 references to write you a letter explaining why your personal character should qualify you to be considered for this scholarship. These personal references must include one teacher from school and one person who is not related to you (students who attend online school can either have a teacher emailed letter or 2 personal references from individuals not related to them.) These letters do not need to include an explanation of your financial need, only an explanation of their relationship to you and a brief description of why you should be considered. These letters must also include the name of the applicant, the name of the personal reference, relationship of the reference to the applicant, and a means of contacting the reference.

Student Scholarship Agreement Grace Chapel Student Ministry or any persons acting on behalf of Grace Chapel (members, employees, attendees, or any other) reserves the right to provide a monetary scholarship or sponsorship for a student to attend an event sponsored by Grace Chapel Student Ministry or being attended by Grace Chapel Student Ministry at the discretion of Grace Chapel Student Ministry. Should a scholarship or sponsorship be awarded to any student for such an event the student is required to fulfill their commitment to attend the event in which they were sponsored or given a scholarship. In the event that the student is unable to attend the event, with the exception of an emergency such as: death in the family (limited only to siblings, parents, grandparents), and severe medical illness as excused by a licensed medical doctor, they will then be required to repay the total amount sponsored or given through scholarship or sponsorship to Grace Chapel Student Ministry or Grace Chapel (members, employees, attendees, or any other). In the event that the student is unable to repay the required amount then the responsibility becomes that of the below signed “Parent/Guardian”. By registering or signing up for this event sponsored by Grace Chapel Student Ministry or attended by Grace Chapel Student Ministry ___________________________(Student’s Printed Name) is committing to attend the “Specific Event” as designated below and agrees to accept the full or partial sponsorship or scholarship amount as required by the “Specific Event” and Grace Chapel Student Ministry.
