Each year, the Health and Safety Excellence Awards honor ARCADIS teams who work collaboratively to steward H&S performance, proactively drive H&S planning and hazard recognition, use ARCADIS H&S tools to foster an atmosphere of incident prevention, implement appropriate control measures for identified hazards and (at times) innovative measures to eliminate hazards, drive positive attitudes toward H&S, and instill a premier H&S culture. The following projects were the 2013 Health and Safety Excellence Award recipients: Gold Project Team Award The Viridian (Agrium) Manitoba Mining Project Teams More than 50 years of product wastes from metals-bearing waste rock and spoils at the Farley Tailings Management Area and Fox Lake mine sites, located north of Winnipeg in Manitoba, Canada, resulted in extremely hazardous conditions for human health and the environment – creating a “moonscape” environment. ARCADIS led the effort to design the technical remedy, consolidate tailings, produce aggregates, procure geosynthetic materials, install multiple cover systems and restore/rehabilitate the landscape. In total, more than 3 million square meters of materials were handled to complete the work, and more than 1 billion gallons of water were treated for discharge to the Lynn River and area lake systems. Nearly 364,000 man-hours were performed without a loss-time incident in temperatures that could dip below -40°C during the colder months. Safety was a primary concern, and the project team designed and incorporated a robust H&S system into their efforts from the outset of the field activities. During the tendering process, ARCADIS weighted safety metrics well beyond the requirements requested by the Province to ensure that we selected subcontractors with a similar safety culture and approach to minimizing risks. Field Level Risk Assessment forms were used on a daily basis, and crews were afforded time during work shifts to continuously evaluate potential hazards. These practices, as well as multiple lines of communication for real-time feedback on safety issues, were keys to the success of the program. ARCADIS not only developed and presented training programs to provide the more than 70 First Nation members who worked on the project with necessary skills, but also addressed their safety concerns by engaging band elders to provide translating and mentoring support. 66000936.0007 | CLB

For ARCADIS, the safely performed work on these sites put us on the map as a premier North American mining reclamation firm, helped to significantly grow revenue and profits for our mining practice in the U.S. and in Canada, and secured our spot as the “go to” provider for Agrium. Silver Project Team Award The Ohio Department of Transportation Culverts Data Collection Team The Ohio Department of Transportation (ODOT) hired ARCADIS in 2013 to identify, inventory and inspect approximately 6,500 culverts crossing state routes within the seven counties of Ohio’s District 11, and to enter data for those culverts into ODOT’s Statewide Structure Management System. To date, the teams have walked more than 500 miles and inspected 1,500 culverts, with zero Lost Work days. The team proactively drives H&S planning and hazard recognition, and uses H&S tools to foster an atmosphere of incident prevention. The well-established flow of communication ensures that field teams are aware that there is someone available to assist them in any situation at any time, and younger crew members are mentored by more experienced field personnel. The project team has also developed a project-specific TIP, and TIPs are performed by each team at least once each week. Near misses are documented and widely shared. Because the project covers such a large geographic area, the HASP includes directions to medical facilities in every area that the teams will enter. All field crews carry an ODOT flowchart for identifying confined spaces and remote culverts. In addition, field crews are encouraged to use Stop Work Authority whenever necessary. Given the rural and hilly terrain, work conducted in proximity to highly trafficked roadways, and the constantly changing Ohio weather, the entire team discusses new areas as they are entered and works to identify new hazards. Crews are encouraged to continually provide feedback on field gear to improve or purchase additional needed items, and to actively reassess JSAs and other components of the HASP. Bronze Project The ExxonMobil Mayflower Pipeline Incident Response Team On the evening of March 29, 2013, the ExxonMobil Pegasus Pipeline ruptured in Mayflower, Arkansas, releasing crude oil into a residential neighborhood and nearby waterways and wetlands. The complex field operations that ARCADIS has conducted to date include rescuing at-risk wildlife, clearing work areas of poisonous snakes ahead of crews, remediating and replacing a stormwater sewer line and water supply line, demolishing severely impacted structures, providing routine maintenance of absorbent booms in wetland/ backwater areas, conducting an extensive groundwater and soil assessment, self-performing a soil removal action, providing a geotechnical investigation of the pipeline area, managing wastes, restoring residential properties, monitoring and sampling sheens, and conducting an assessment of biota, sediment, soil and surface water in drainage ways, wetlands and nearby Lake Conway. Health and Safety planning and management went into effect even before AUS boots were on the ground, and they have been a primary focus of the team throughout the life of the project. The team has successfully and safely worked under extreme scheduling pressures and intense scrutiny, and has religiously used the Safe Work Planning Guide developed recently by the ARCADIS ExxonMobil Global Account Team. Observation findings and near-miss investigations have helped to coach team members, improve JSAs and procedures, and prevent future hazards. Dedicated, full-time H&S field supervisors have been onsite during response and follow-up remediation activities. But perhaps the most important element of all is our top-notch, safety-focused staff – all vetted for their level of experience and past safety performance prior to deployment – who have given up their vacations, holidays and weekends to help clean up and restore Mayflower. 66000936.0007 | CLB

ExxonMobil recognized this achievement by presenting the team with an award citing “Outstanding Performance Recognition for flawless execution of emergency work on the Mayflower Pipeline Incident Project.” Non-Project Team H&S Excellence Award The CSX Team The CSX Team maintained its focus on H&S under intense conditions during 2013. They lived “TRACK to Zero,” with no recordable injuries. Rail yards are extremely unforgiving places, where there is the potential not only for injuries but also for fatalities. Add in the numerous incident response and recovery (IRR) efforts that the team completed in 2013, and the fact that this work was performed safely is even more impressive. IRR work was completed in the intense heat of summer and the bitter cold of winter. The team dealt with a number of chemicals, each with unique hazards that had to be mitigated. This work was performed 24/7, including holidays. Despite fatigue and harsh conditions, the team still kept H&S first in all they did. In addition, the team provided H&S oversight not only for ARCADIS, but for all of the responding contractors and CSX employees. Feedback from our client has been fantastic. Innovation by Design Award The BP Foxglove Michigan Site Security Team The BP Foxglove Michigan Team developed an innovative Site Security Evaluation System for assessing and reducing security risks to our staff (and subcontracted staff) working in and around the City of Detroit, Michigan, where crime rates are high. Fatal and non-fatal shootings, robberies, and beatings were happening at, or very close to, several of the sites. In-depth discussions of the potential for security-related safety issues were included in kickoff meetings for field work, and communication from the field staff regarding site conditions was reinforced. The team developed an innovative hazard ranking system that rated sites according to their level of security and prescribed safety measures – which were added to the site-specific HASPs. Using this Site Security Evaluation System, the BP Michigan team has been able to successfully meet its contractual and regulatory requirements while keeping all of our staff safe. Approximately 270 field events have been performed without serious incident or injury. At least eight documented uses of Stop Work Authority have been reported due to the heightened awareness exercised by our field staff. The only loss to date involved a stolen water level meter and tools. The ranking system has since been incorporated into the national BP portfolio as a HASP Addendum for assessing and mitigating security risks. Last year, the ARCADIS-designed Michigan Site Security Evaluation System was presented to BP as a Best Practice. BP was impressed with our approach and forward thinking, and has shared the system as a Health and Safety Best Practice with its other consultants.

Training Qualifications We develop engaging and customized computer-based training courses, manuals, and other materials for our clients’ Health and Safety programs. Our staff members work with client personnel to develop content, and then takes that content to create highly engaging, effective online training modules to inform and educate a target audience. To do this, we assemble teams of ARCADIS subject matter experts, training instructors, instructional designers, communication specialists, and graphic designers to develop training programs to suit the need at hand. 66000936.0007 | CLB

Specifically, we design and deliver online e-learning modules, instructor-led sessions, webinars, and other educational tools to more effectively train people on a variety of subjects such as health and safety topics, environmental compliance, and computer systems training, to name a few. We create interactive training programs to help clients educate personnel, and provide support throughout the implementation process. We work with clients to make sure these modules are compliant with their learning management systems (LMSs) and work with our client’s personnel from the testing and launch phase all the way through completion tracking and reporting. We can even develop, construct, and maintain LMSs in behalf of clients who do not have a system already in place. Our training services include the following:

IDEA is ideal for managing environmental information and data at portfolios of sites such as offices, retail locations, distribution and transportation installations, and light industrial facilities that may not have dedicated environmental or engineering staff. IDEA is a scalable, easily managed environmental information management system for organizations that are trying to improve and demonstrate environmental performance while lowering operational costs. Representative Training Experience National Health and Safety Training Program via E-Learning Modules and Reinforcement Materials for Confidential Global Services Company – ARCADIS has assisted with completing a national safety training program for a major client. The program entailed developing a series of occupational health and safety e-learning modules and associated training handouts. The program focuses on a series of safety topics such as slip, trip and fall elimination, the 5S process, and safety meetings. We also worked with this client on their new Employee Safety Handbook, which covered a variety of health and safety topics, such as general safety rules, safety signs, slip, trip and fall prevention, lockout/tagout, personal protective equipment, emergency evacuation planning, ergonomics, safe equipment operation and tool use, hazardous materials handling, bloodborne pathogens awareness, incident reporting, and environmental safety. We provided the following services on this project:

We continue to serve this client by updating/revising the modules, maintaining the LMS to ensure that users can easily access the training modules, and providing tracking reports back to the client every week. Our proposed team members bring all the skills necessary for the wide variety of topics indicated by the City’s RFP and have a proven track record of working successfully together on similar projects for similar clients. The organization chart to the right provides an overview of our team’s organization, which we have created in direct response to the City’s needs as articulated in the RFP. Tad Rogers, PE, CCCA, CDT - Water Treatment Mr. Rogers has extensive experience in water and wastewater infrastructure improvement projects, serving primarily municipal and governmental clients as a professional engineer in the design and construction administration of multimillion-dollar projects. His responsibilities have included technical research, design engineering, and preparation of construction drawings and specifications in compliance with local, state, and federal regulatory agencies. Mr. Rogers has specialized technical expertise in the design of water and wastewater projects including treatment, storage, supply, and conveyance. ARCADIS developed and supported the implementation of a comprehensive environmental, health and safety program for NYCDEP. The program development addressed the needs of a range of NYCDEP Bureaus including its Bureau of Engineering, Design and Construction responsible for capital projects in the water supply and wastewater systems for the City of New York. The majority of the project efforts focused on the Bureau of Wastewater Treatment. The Bureau’s 14 plants, 95 pump stations and 2300 employees treat a billion gallons of wastewater daily.Health and safety programs including confined space entry, machine guarding, fall protection, hazard communication, lockout, arc flash and electrical safety were developed and implemented. Training was developed and provided to DEP staff.Environmental programs including air permitting, waste management, SARA reporting, storm water, and storage tanks were developed and implemented. ARCADIS use the IDEA data management system to assist Comcast Corp. with the management of EHS data including documents, permits, regulatory communication, and training. IDEA also maintains current inventory data for lead-acid batteries, storage tanks, and emergency generators. The centralization of this data empowers local contacts to keep local information updated. Furthermore, it reduces the overall number of hours dealing with administrative tasks such as communicating local changes, updated regulatory requirements, or staff information. Since 2007, the client has seen continually significant cost savings each year using IDEA. Some of the more effective features include: e-mail notification when a new storage tank, generator, or operating permit is added so the project team can review for SPCC and EPCRA requirements, permit renewals, budget changes, and various other regulatory requirements; documents linked to inventory under a site so it is easy to pull up a history of documents for a single piece of equipment; and, reminders triggered off of dates of certain kinds of files, like SPCC Plans, when the plan needs to be renewed. Created an organizational strategic plan - ARCADIS worked with CORPUD senior management staff to provide a guide for evolving policies, practices, and programs consistent with CORPUD’s desire to become a more effective and sustainable utility and achieve selected outcomes of EUM. EUM was used as the basis for the evaluation of the



Department to identify gaps where improvement work should be undertaken. Developed business process mapping training materials for City staff - In conjunction with the City’s Organizational Development and Training Group, ARCADIS developed the foundational training elements for business process



mapping/documentation. Completed a fleet management study - ARCADIS evaluated CORPUD’s existing fleet organization, processes, policies, and practices. CORPUD’s vehicle fleet size and mix, maintenance practices, facility locations and their



impact on travel time, and greenhouse gas emissions (GHG) were assessed to reduce costs and GHG emissions. Conducted business process mapping for CORPUD’s Meter Operations group -ARCADIS documented existing and recommended future business processes for CORPUD’s Meter Operations group to ensure that CORPUD can deliver its current and future Meter Operations services consistent with industry best practice.

Conducted an assessment of the potential reorganization of the City’s Utility Billing group.-ARCADIS performed a highlevel assessment of the potential relocation of the City of Raleigh Utility Billing group within CORPUD to provide more efficient and effective services to public utilities’ customers and stakeholders. San Francisco Public Utility Commission Strategic Business Planning and Leadership Development │ San Francisco, California ARCADIS is currently engaged with SFPUC’s WWE to provide organizational effectiveness services centered on the utility’s strategic business plan, change management and leadership development. We are working with WWE managers to update the existing strategic business plan and develop an implementation plan that will result in better adoption of the plan at all levels in the organization. The implementation plan is focusing on improving “processes, communication, and tools.” We assessed the impact of the current strategic plan through an extensive survey, coupled with meeting with senior staff members. We then hosted a 3-day Leadership Symposium with the Division’s top managers that focused on team building, strategic plan implementation, Myers-Briggs personality assessments and leadership development. To increase awareness and involvement of managers and staff in the strategic business plan, ARCADIS is developing a Communications Plan which includes key messages for targeted audiences, internal engagement activities, and collateral materials. To put the strategic plan into action, we will work with WWE to estimate the budgetary impact of implementing the plan in FY 14/15. We will also provide executive coaching to individual managers with a focus on leadership, delegation, mentoring, and change management.

The purpose of the assessment was to identify gaps, if any, in KSAs, and to provide input into a Mentoring Program Implementation Plan. The staff assessment task consisted of:



Creating Role and Responsibility Sheets – Working collaboratively with Utility staff and management, ARCADIS developed prototype Role and Responsibility Sheets for the primary program and project management roles envisioned for the future. The sheets listed general requirements, KSAs, and experiential or educational pre-



requisites for each potential role. Reviewing Current Position Descriptions – ARCADIS reviewed current position descriptions applicable to the staff selected for potential program and project management roles. The purpose of the review was to document features



of the position descriptions that either promoted or inhibited the intended roles. Assessing Staff – ARCADIS met with each staff member and performed an assessment with respect to the KSAs required for the desired program and project management roles. The assessment was administered through a brief up-front questionnaire and a more detailed individual interview. Each staff member was asked to indicate their level of interest to pursue an expanded program or project management role and to participate in a formal mentoring



program. Performing a Gap Analysis – ARCADIS compared the current attributes of staff with the requirements of each role, and prepared a summary gap analysis memorandum. For each staff member, key areas of opportunity discovered through the gap analysis process were categorized as high-, medium-, and low-priority for potential training/mentoring enhancement.

Proposed Approach and Schedule 1. Review of the City’s Risk Management Program A review of the city’s risk management program as it pertains to the development of a safety program for the Department of Public Utilities would include the following:

The outcome of the review process will be an understanding of the City’s risk management requirements and expectations. These will then be integrated during the safety program development where applicable. We anticipate communication with Risk Management staff as needed throughout the project. 2. Review of Department operations, becoming familiar with staff and current daily operational/work practices ARCADIS recommends a highly interactive approach to safety program development, beginning with developing our understanding of the Department operations, staff, issues and needs. This interactive approach is also important for the overall program as our experience has shown that a high level of involvement significantly improves the implementation and effectiveness of safety programs and procedures, and builds the foundation for a sustainable program. The ARCADIS team benefits from its unique experience with and understanding of water and wastewater operations. This will allow the team to become quickly and actively engaged with staff to learn the specifics of your operations, as well as the needs, concerns and preferences of Department staff. 3. Development of Detailed Department Health and Safety Plans, Job Safety Analysis and Safe Work Procedures Consistent with Existing City Policies For procedures to be effective they must not only be technically complete, but also understandable and practical for the user. Our interactive process has proven effective in achieving this goal. We will begin with draft plans and procedure, based on regulatory and City requirements, our safety experience, and knowledge of your operations. A series of interactive workshops are proposed to review and obtain input from Department staff. We have found workshops to be efficient and effective in that questions and concerns can be immediately addressed and answered and differing opinions or alternatives can be resolved without a lengthy document review process. The workshops are an effective forum for getting the Department input necessary to make the procedures practical for Department staff. An additional benefit is that by participating in an interactive process the participants gain an understanding and retention of the procedures far beyond what would be achieved in a traditional classroom training program. These knowledgeable workshop participants can be an excellent resource for ongoing support of the procedures. We will follow the Department or City’s process for review and approval of procedures, or assist with the development of a process as needed. Procedure development, workshops, review and approval will require involvement of Department staff. If not adequately planned this could be time consuming and disruptive. We’ll establish a work flow together that maintains steady progress at a pace manageable for Department staff. Some plans and procedures may be a higher priority based on risk or the Department’s needs, or take longer to develop due to length and complexity such as lockout/tagout or confined space entry. The development schedule will be established with input from Department staff. It is recommended that the development process start with a few procedures of lesser complexity. We have found that starting with simpler programs helps to quickly build success and momentum that contributes to project success. Upon completion of plan and procedure development it is recommended that the Department establish a process for review and updating of procedures, including the management of change in operations or processes. We can assist with the development of this process if needed. Hazard Analysis We have developed numerous types and styles of hazard assessments related to processes, equipment, and human behavior. These assessments typically include plans that highlight equipment maintenance requirements, employee training, operational guidelines, and inspection requirements. We use industry-recognized methods to perform hazard analyses of processes, operations, equipment, and tasks based on site-specific conditions. The hazard analyses identify relative health and safety risks based on probability of a deviation from normal operations and the severity of the impacts that may result from the deviation. initiated for the process, operation, or task. Where the hazard assessment determines that an unacceptable residual risk remains, the team will make recommendations or develop a prioritized risk management plan. The purpose of the hazard analysis is to ensure that processes, operations, equipment, and tasks have been analyzed in a methodical and structured manner ensuring that all aspects have been evaluated related to risk. The below methods may be used for hazard analysis.



Task Specific Hazard Analyses – Conducted using a standard Job Hazard Analysis (JHA) methodology, similar to the methodology outlined in OSHA 3071 manual. JHAs will identify potential EHS hazards associated with the task and operations. Once hazards are identified, appropriate engineering controls and administrative controls will be identified; PPE will be identified to either reduce or eliminate the hazard where such controls are not feasible. A Personal Protective Equipment (PPE) hazard assessment will be included in each JHA to evaluate



and confirm that the PPE used for the task is adequate and properly used. Equipment Hazard Analyses – We develop a methodology to evaluate risks associated with equipment consistent with American National Standards Institute (ANSI), American Society for Testing and Materials (ASTM), and other standards. The equipment hazard assessment methodology would be targeted at assessing the hazards of mechanical equipment not included in an inspection program, such as conveyors or heater pumps that may present a hazard from poor maintenance, improper operating



procedures, design deviations and/or alterations to the intended design. Process Hazards Analysis - We use a standard process to “risk rank” the action items arising from hazard assessment activities based on the probability of the occurrence and the severity of consequence(s) to determine the risk of the deviation identified in the hazard assessment. Following hazard analysis, we can assist in prioritizing necessary actions to mitigate or remove risks. A corrective action tracking process may be established to manage the completion of recommended risk reduction actions. The process will determine the relative risk of each follow-up item and determine the priority level of each item.

4. Development of Detailed Department Computer-Based Training Modules Which Meet OSHA Requirements We can design e-learning modules and other educational tools to effectively train your people and teams. We can create interactive, online programs to help you educate your staff, and support you throughout the implementation process. Our staff develops e-learning modules tailored to meet our client’s specific training needs. We make our client’s elearning modules effective and engaging by using site- and project-specific information, photographs, videos, animations, live website content, quizzes, and a variety of interactions. Our modules can be linked to internal Learning Management Systems (LMS) so you can track training progress. They can also be deployed to a website, presented during a webinar, sent via e-mail, or delivered on a CD or DVD. Currently our staff participates in the U.S. Technical Advisory Group (TAG) for the ISO 45001 Committee for Occupational Health and Safety Management. Our approach is built on the City practices and policies by working with the City and Department to identify and include proven practices and systems, such as those in the OSHAS 18001 and ANSI Z10 standards, in the Accident Prevention Plan. These standards are built on the PlanDo-Check-Act (PDCA) cycle for continuous improvement and can provide an effective model for the Department’s plan. In addition to the systems for accident prevention, our team can address specific accident prevention issues such as ergonomics and machine guarding.
