 HEALTH & SAFETY ALERT What happened: On 11th of July 2018 at Finsbury Circus, an Armorguard Sitestation, was being moved using a pallet truck. While pushing the pallet truck forwards, one of the wheels hit an indentation in the ground. The ‘Sitestation’ became unstable, toppled, and fell on an employee who tried to stop it from falling.

The IP sustained significant injuries including a double fracture to pelvis and a dislocated patella. Findings:

Sitestation

• The ‘Sitestation’ was being used as a storage unit, it weighed 245KG alone and contained an additional 250kg of content inside • The ‘Sitestation’ was loaded inappropriately (top heavy), with the bulk of the load above the centre of gravity (C of G) • A pallet truck was being used as it was difficult to move the box on the fitted wheels, this was an unstable method of moving the box • There was insufficient application of hazard identification and risk analysis overall in the execution of this task • This is the third similar incident that we have seen on the project in the past two years Contents of Sitestation

Actions: Initially, movement of site storage/work boxes was prohibited across the Project. This can now be waived provided the following action is undertaken: • Inspect all ‘Sitestations’ and other site storage/work boxes to assess position of contents • Ensure staff follow manufacturers instructions in the use of all equipment on site • If this equipment must be moved, ensure that the load is stored below the C of G • If the load cannot be stored below the C of G, empty the load before moving • Provide signage for all site storage/work boxes, reminding personnel of safe use/loading • Undertake pallet truck awareness training to ensure the equipment is being used correctly
