Inner Strength Mental Health FEE SCHEDULE 1INITIAL

VISIT - $220 - 60 minute evaluation

FOLLOW UP VISIT - $120 - 20 minute evaluation ** $170 - 40 minute evaluation - medication management and therapy $200 - 60 minute evaluation - medication management and therapy ** Please let us know if you believe you will need an extended appointment or if you wish to have therapy, we will make sure to schedule you for a longer appointment. We do not want to run over and take up the next persons appointment time. ** If you feel that you need a longer appointment time please let your provider know and this can be discussed. Please be advised that an extra charge will be necessary for a longer appointment time.
 FOLLOW UP TELE-VISIT - $ 120 - 20 minute evaluation. Follow up visit times and fees are the same as above. 
 $120 - 10 minute evaluation in lieu of late, reschedule,
 canceled or missed appointment charge. Essentially a medication refill. TELEPHONE CALLS - If phone calls become frequent or longer than 10 minutes and/or include information that needs to be discussed in an actual visit, you may be charged per call. Fees are subject to the length of call. Fees will begin at $30.00 Your card on file will be charged. EXTRA SERVICES TREATMENT PLANS - If you wish to have extra services along with your scheduled appointments, please ask. We can customize services to meet your needs. Examples - daily, bi-weekly, weekly, monthly visits, emails, phone calls, text messages.

Inner Strength Mental Health FOLLOW UP VISIT WITH FORMS - $150 - 30 minutes.
 If you require forms to be filled out, please make an appointment for 30 minutes. We will fill out the form with you in the office during your appointment. Please fill out all personal information prior to the appointment. Disability: I do not perform assessments or complete paperwork for disability claims.
 Please note — You may avoid this type of follow-up — IF you request that the recipient accepts a copy of the relevant office visit note in lieu of a form. URGENT VISITS - $100 in addition to the appointment fee.
 An urgent visit is a visit that can not wait for the next available appointment slot and must be fit into the schedule.
 Please remember - If there is a life threatening emergency - Call 911 or go to your nearest emergency room. FORMS FILLED WITHOUT AN APPOINTMENT - 1-3 pages - $ 20.
 $ 2 per page thereafter. Paperwork to be filled out may be scanned into a document and sent via the patient portal messaging system or can be given to your provider when you come in for an appointment. Paperwork may be returned to you via the patient portal and/or you may schedule a pick up via the portal messaging system. Please allow up to 5 business days for completion.
 Please note — You may avoid this charge — IF you request that the recipient accepts a copy of the relevant office visit note in lieu of a form. AFTER HOURS CALL (10PM TO 5AM) - Urgent calls will be returned as soon as possible. Please remember - If there is a life threatening emergency Call 911 or go to your nearest emergency room. Non-urgent calls will be returned early in the morning. FEES FOR RECORDS Copies of records are charged by page. Fees for copies are $0.20 per page and are due in advance. Where records are to be released to a third party, we must have an authorization to release the information signed by you and any other party involved in treatment. If the client is a minor child, parent/s and/or legal guardian/s must sign for treatment and for release of records.

Inner Strength Mental Health Fees are subject to change. You will be notified of changes. If your payment method of choice is not listed, please ask, as we will attempt to provide as many payment options as possible.
