The Department of Administrative Services (DAS) Facilities Division requests written quotes for the scope of work described in this Request for Quote. Any reference to brand names is meant as descriptive not restrictive. Alternate bids will be considered on products, which meet or exceed products indicated on quote. Vendor is to mark clearly alternate proposals. “OR EQUAL”: Any brand name listed in the specifications as “or equal” or “or equivalent” shall establish the minimum requirements for quality, utility, durability, function, purpose, etc. Other product brands may be offered that are equal to or better than the product brand name. Bidder may show cost differences, alternates and options in the space provided in the quote. This clause is not meant to be restrictive, but to set the minimum standard. AGENCY SHALL DETERMINE, IN ITS SOLE DISCRETION, WHETHER A PRODUCT OFFERED IS “EQUAL.” When the designation is “or equal” or “equivalent” Agency shall make its decision after Bid Closing.

ATTACHMENT NO. 1 GENERAL CONTRACT PROVISIONS I.

PROJECT DESCRIPTION A. Contractor shall provide Fire and Security Alarm Monitoring at selected building sites in the Central Oregon Region (Salem) area (see Exhibit B – Site Locations and Pricing).

II.

GENERAL WORKING CONDITIONS A. 24/7 fire alarm and security monitoring service at selected building sites must be in place by July 1, 2015. B. Contact persons designated for this Contract are listed in Exhibit D – Contract Contact Information. C. All electrical work shall be performed in strict compliance with all local, state, and federal codes as well as the DAS Electrical standards online at (http://www.oregon.gov/DAS/FAC/docs/1256211.pdf).

III.

SPECIFICATIONS A. Owner’s alarm panels shall provide signals to Contractor’s equipment through generic non-proprietary dialers. B. Contractor shall monitor fire alarm and or security signals sent by various mediums. C. Upon receiving a signal, Contractor shall make reasonable effort to notify the Owner or designated individuals identified by Owner as the proper party under the circumstances encountered. D. Contractor shall provide monitoring services continuously to the building listed on Exhibit E – Monitoring Site Locations and Pricing, on a twenty-four (24) hour basis, seven (7) days per week, 365 days per year. E. Owner’s system(s) shall provide the necessary working signal to the monitoring service provider. F. Contractor shall provide one time initial set up and programming of monitoring from the signal provided by the buildings’ system(s) if applicable G. Building information is listed in Exhibit E – Monitoring Site Locations and Pricing. H. Contractor’s work shall comply with all federal, state, and local laws, codes, regulations and ordinances applicable to the Work and the Contract.

IV.

SCHEDULING OF CONTRACT A. Prior to the start of any work under this Contract, Contractor shall contact DAS EAM’s Electrical Manager of the Maintenance Section, as listed in Exhibit D, to schedule a minimum of seven (7) days prior to start of work. B. Any work to be performed on site shall be scheduled during normal office hours (8:00 a.m. – 5:00 p.m.) EXCEPT as noted in these General Contract Provisions, to exclude weekends and State holidays unless specifically requested by Contractor and approved by State.

V.

CONTRACTOR REFERENCES A. Contractor shall provide a list of references for a minimum of three different customers’ projects they have completed within the last three (3) years in similar size and complexity.

C. The references may be checked to determine if they are supportive of the Contractor’s ability to meet the requirements of this project. D. Contractor must provide references who can be contacted regarding the quality of workmanship and service provided. E. DAS reserves the right to select and investigate any reference whether or not furnished by the Contractor, and to investigate past performance of any Contractor with respect to its successful performance of similar projects, compliance with specifications and contractual obligations, its completion or delivery of service on schedule, and its lawful payment of suppliers, subcontractors, and employees. F. DAS may postpone the award or execution of the work after the announcement of the apparent successful Contractor, in order to complete its investigation. G. DAS may reject a Bid if, in the opinion of DAS, overall reference responses indicate inadequate performance. H. DAS will make three (3) attempts to contact the references from the list provided by the Contractor. I.

Each reference contacted will be asked the same questions, including but not limited to: (1) quality of service and installation; (2) meeting scheduled deadlines; (3) responsiveness to reported problems, including orders and billing; (4) how well Contractor met the specifications of the project; and (5) whether the reference would choose to hire the Contractor again.
