The wrong questionnaire spreadsheet was posted to the web site and attached to the initial email. The correct questionnaire spreadsheet will be posted to the web site and attached to the email sent with this amendment.

Question:

The due date in section 3-1 General does not match the date at the beginning of the RFP?

Response:

Section 3-1 has now been changed to read:

GENERAL Please complete the enclosed Excel Questionnaire titled the “RFP 15-096 JCCC Benefits Administration and Online Enrollment Services Questionaire 3-26”. Additional detail and information about JCCC’s current benefits administration process is contained within the Excel document. Proposal Due Date: Monday, April 14, 2015, by 3:00pm. Effective Date: Open Enrollment begins in April 2016 Insurance Benefits Policy Anniversary is June 1, 2016 Questions Due Date: All questions are due to Gary Cromer by 5pm on Friday, April 3, 2015
