St. Therese of Lisieux Little Flowers & Daughters of the King Girls’ Club We are a fun & faith-filled club for K-8th grade girls—& their moms— to enjoy uplifting mom-led meetings that bring our Catholic faith alive through creative, virtue-building activities; all while seeing the beauty of strong women saints as role models & cherishing our authentic femininity, chastity, and sanctity of life.

Enjoy family-bonding club traditions: St. Therese Sunday, Mom/Daughter Tea, & Daddy/Daughter Royal Ball Know that ALL of your dues stay here for 100% Catholic programming, meeting supplies, & service projects (We hope to have a Babysitting Co-op or hired sitters’ service to help moms enjoy meetings with their daughters)

Parental Permission for Child Participation in Little Flowers/Daughters of the King Girls’ Club We permit our daughter(s) to participate in the Little Flowers/Daughters of the King Girls’ Club (a.k.a. “The Club”) meetings, outings, and events at OLMC Parish on campus and off-site from Sept. 2014-Aug. 2015. We understand that attendance at off-site outings & events is optional, may have additional cost and permission slips, and will require transportation provisions made by us as the parents/guardians. We understand that The Club cannot provide transportation. If outing or event is held outside of regular club meeting time, we (the parents/guardians) must attend along with our daughter(s.) We agree to submit a Medical Emergency Waiver**, Media Release, & any other required club paperwork, for each daughter, no later than Sept. 12, 2014. If we use babysitting services, we agree to trade time for the Babysitting Co-op services or to pay fees for hired sitters. We agree not to send or bring a sick child to a meeting, babysitting, outings, or events and understand any child attending must be 24 hours free of fever, vomit, sore-throat, or extremely runny nose. The Club activities are communicated via the annual club calendar, regular email updates, on The Club website, etc. We, the parents/guardians of the undersigned minor(s), hereby consent to hold harmless the Little Flowers/Daughters of the King Girls’ Club, Our Lady of Mount Carmel parish, and/or the Roman Catholic Diocese of Lafayette-in-Indiana, Inc., and any and all volunteers or employees thereof, for any accident, injury to our child or children or occurrence arising out of, or in connection with The Club meetings, outings, or events or any club-related activity. We are aware that the adult volunteers working in The Club have attended the Working with Minors Protocol Training and have met diocesan standards for working with youth; and agree to keep our own protocol-training updated to attend meetings or help with babysitting. We will assist, to the best of our ability, with a meeting (if Co-Lead requests) and volunteer time to help with a club event.
