Membership Policies Welcome to the Stroller Strides family! We are glad you chose us for your fitness needs and hope you enjoy our Stroller Strides Program! Stroller Strides is a great way to get in shape, meet other moms and their children and to gain support as a parent. We would like to take this opportunity to remind you of our policies and how to contact us with any questions or concerns you may have regarding your membership. Our goal is for you and your family to have a great fitness experience with us. Unlimited Monthly Membership Benefits: - Classes can be attended at any of the Fayetteville Fit4Mom franchise locations (Fayetteville and Spring Lake). - Membership bag, (2) Resistance Tubes, lots of free samples and deals from local and national partners. Billing Procedures: -We accept Visa, Discover, and MasterCard as forms of credit payment. Checks are accepted and must be made payable to “Sweat To Fit Fitness”. Membership fees will be charged on the 1st of each month. -Funds that are denied for any reason will be subject to a re-processing fee. You can manage your account (and credit card information) by logging onto: https://fit4momfayetteville.frontdeskhq.com Cancel/Hold Policy: Members can cancel or hold their membership written notice. Cancelations must be given to Hannah Ambrozewski with notice, prior to their next payment due date. Your request for cancellation must include identification on when you would like your cancellation to take effect. No refunds will be given for dues once it has been paid. You may put your membership on hold for a maximum of 4 months each year (12 month period), not to exceed two different hold periods per year. Please note that there will be a $10 processing fee charged to your credit card on file for the hold period. This is a one-time fee, charged per each hold period, and must be paid prior to the hold start date. If your membership is cancelled, and you wish to re-enroll, you will be charged half ($25) of the $50 registration fee to reactivate your account. Your monthly dues will be set to automatically start back up at the end of your hold period. It is your responsibility to make sure we have received the written confirmation that your status change request (hold end date) was received and will be put into place. Our instructors cannot authorize hold or cancel requests, either verbally or in writing. Holds or cancellations will only

take place for upcoming months. We will not go back and credit dues already paid. Memberships are not transferable to other Stroller Strides locations outside of the Fayetteville franchise or to other persons. **Please contact Hannah Ambrozewski at [email protected] in order to cancel/hold any memberships. Stay Informed: You will be added to our e-mail list to receive important updates and receive our monthly newsletter. You can also “like” our Facebook page AND join our Facebook Group (titled Stroller Strides of Fayetteville) to network with other moms, RSVP for events such as Playgroup and Mom’s Night Outs, special events and be aware of any updates for classes including inclement weather class locations. Refer a Friend: Refer a new member to SS! If they join as an unlimited monthly member, you will receive 20 % OFF your next month of membership! To receive this discount make sure that they identify you (by name) in their new account form under the question: “How did you hear about us?”. So, start recruiting! Got Visitors: Have family or friends in town? Is hubby off of work for the day? Bring them to class FREE of charge! If you have an extended stay guest who will participate in class, they may be subject to a $5 per class charge. Family members will be charged $5 per class after two (2) weeks of attending class and not to exceed three (3) weeks, before a membership is required to attend. Stroller Policy: For your child's safety and your peace of mind, please adhere to our policy and assure her that she can get out after our last and most important stretch – a smile. Prices/Policy Changes: Pricing and policies are subject to change. You will be made aware of notification changes that will affect your current and/or future payments via email account listed in your Fit4Mom Fayetteville account.
