Michelin 2018 Annual Enrollment Guide Michelin asked Conduent to create a new look and feel for its annual enrollment guide, utilizing the company’s new brand guidelines (imagery, colors, fonts). The result was an engaging format with eye-catching imagery, more concise copy and a handy resource magnet that employees could detach. At the end of the enrollment window, more than 93 percent of the eligible population enrolled, with nearly 75 percent of total enrollments using the online benefits portal— an increase of nearly 4 percent from the previous year.

Take Advantage of Condition Management for Chronic Conditions and Save on Expenses. Call 1-XXX-XXX-XXXX to talk to a Condition Management nurse, who can help educate you about your condition and assist with lifestyle changes. If you or your eligible dependents are enrolled in a Michelin medical plan and are diagnosed with one of five covered conditions, a Condition Management nurse can help you understand and receive the Basic Care Requirements (BCRs) for your condition at no cost to you. Covered conditions include: asthma, chronic obstructive pulmonary disease, congestive heart failure, coronary artery disease and diabetes. (Eligibility rules apply.) Be sure to review the updated monthly premium rates based on the deductible option you choose and the family member(s) you choose to cover. (See pages 2-3)

You may upload your dependent eligibility documents, such as marriage certificate, birth certificate, official hospital records for newborns only,* adoption certificate and copies of official court documentation. To upload a document, simply click Document Upload in the upper right corner of the PSC Home Page. If you are using a tablet or smartphone, you can take a picture of the document and upload the picture from your device. Once the document(s) are successfully submitted, you will receive a notification that the upload is in process, and you can check back in an hour to confirm completion.
