Aegis Therapies has developed a suite of comprehensive core, featured and specialty programs and services to help meet the unique needs of patients with a variety of conditions. Our services fall into two main categories – therapy and wellness – and all are designed to enhance patients’ lives by maximizing functional ability and increasing independence. Our therapists utilize the latest therapy treatments and equipment to execute evidence-based therapy plans tailored to the individual. We embrace a “Restore, Compensate, Adapt” model of care. We consider not only what can be improved in the patient’s functional abilities (restore), but also what can be changed in the patient’s environment (compensate and adapt). Our therapists consult with nursing staff and the patient’s family so every therapy program is specifically tailored to the patient’s needs and potential. DOCUMENTATION
Aegis Therapies has an in-house review system, with checklists and tracking logs to monitor and track patient care orders and certifications, to ensure compliance is maintained. Additionally, the on-site manager, with oversight from the Area Vice President, ensures that Aegis systems for tracking and monitoring certifications and physician orders comply with facility/nursing systems at all times. In the event a problem in this area is uncovered, the Aegis Compliance Department works directly with the on-site manager, Area Vice President, Compliance Liaisons and Clinical Directors to provide training, problem resolution and tracking of compliance related issues.
Aegis Therapies has fully implemented and utilized electronic clinical documentation since 2009. The Casamba SMART documentation program, though an off-the-shelf program, was significantly modified with the medical reviewer in mind. Aegis created starter phrase libraries to help cue the therapist to document using medical necessity and skilled services language. We reformatted the printed documents to assure that the necessary information needed by a reviewer to make payment decisions is at the top and very noticeable. We ensured logical flow for easy decision-making on the reviewer's part.
Compliance Monitoring
Aegis Therapies has developed an extensive documentation training provided through eLearning. The training takes the therapist through the concepts of documenting medical necessity and skilled services. Additionally, it has a module on documentation auditing. Enhancements currently under development will result in converting this training into modules that will be completed by all staff on an annual basis to ensure constant focus on documentation quality.
At Aegis Therapies, monitoring and compliance are part of every manager’s and therapist’s performance plan. Managers are required to submit written documentation of any compliancerelated concerns to their supervisor and VP Compliance. There is a process in place for obtaining and maintaining updated State Practice Act information. An automated licensure tracking system is in place to ensure compliance. Licensure renewal dates for all disciplines are sent out each month to all Aegis managers. Each Area Vice President, along with credentialed auditors, must complete mandatory audits each quarter to ensure compliance with regulatory standards. These audits, and feedback, are shared with the original documentation author as another layer of training. This process is automated to encourage completion and compliance, and results are readily available in report form. Quality Assurance For Quality Assurance, Aegis completes Facility Visit reports and Survey Readiness assessments. Combined, this ensures the quality of services Aegis Therapies provides to patients and the facility, identifies risks for exposure, provides trending reports, and creates follow-up/action plans. Through these activities, Aegis ensures that we are aligned with facility quality management programs by incorporating two key standards: Quality of Care, and Regulatory and Company Requirements. • Q uality of Care: Specialized rehab services are provided to all patients who require them. These services address Quality Indicators, and serve to enhance and maintain the patient’s highest functional level. • R egulatory and Company Requirements: The rehab department supports the goals and objectives of the client facility, and complies with state and federal clinical practice guidelines and regulatory requirements.

13 INTERNAL AUDIT PROCEDURES
DENIALS MANAGEMENT
To assure quality documentation and billing integrity, Aegis Therapies requires documentation review on a regular basis. Our documentation reviews are conducted both randomly and targeted toward specific “outliers.”
Aegis Therapies is one of the few rehab companies to employ a dedicated Outcomes and Reimbursement team.
Documentation reviews are completed to identify areas of risk and areas of opportunity. While Aegis Therapies has policies that ensure documentation reviews are performed monthly at each Aegis Therapies facility to achieve a minimum number of reviews for each quarter, the expectation is that each facility is aware of their risk areas and uses documentation review as a tool to control their risk. Trends are monitored at the corporate, area, facility and therapist level with plans of correction developed as needed. Documentation reviews are conducted by Aegis Credentialed Documentation Reviewers. The credentialing program was developed and is trained by our Senior Clinical Claims Review Specialist who was a Medicare Medical Reviewer for over nine years prior to coming to Aegis Therapies. Currently, we have 325 credentialed reviewers. Aegis Therapies has created an electronic auditing program that displays the auditing form alongside the document that is to be audited. The auditing questions focus on the key areas that a Medical Reviewer looks for in determining medical necessity and skilled services.
This provides our customers with knowledgeable and experienced resources who are highly involved in government affairs and dedicated to strengthening our relationships with fiscal intermediaries. Our Outcomes and Reimbursement managers are responsible for the following: • E stablishing positive, proactive contact with Fiscal Intermediary (FI) and Medicare Administrative Contractor (MAC) or strengthening existing contact • D etermining FI/MAC requirements for documentation of therapy services (i.e., evaluation, physician communications, progress notes, monthly summaries, discharge summaries, etc.) via monitoring of Local Medical Review Policies, bulletins, newsletters and website • D etermining FI/MAC requirements for Appeals Process (i.e., written vs. oral; Reconsiderations/ Re-openings, Fair Hearings, Administrative Law Judge reviews, etc.) • Overseeing/managing entire Appeals Process, including all timelines • Producing trending analysis of denials received from FI/MAC • T raining therapy staff regarding documentation standards/ approaches as related to denials trending and FI requirements • T raining therapy staff regarding quality improvement (QI) processes, such as peer review of documentation • A ll denials reported to Aegis Therapies are entered into a Denials Tracking System (DTS). Aegis Rehab Outcomes are designed to ensure that quality services and positive outcomes are delivered to each patient by allowing clinicians to identify best practices. Through our Aegis Rehab Outcomes Report Cards, patients are able to see their progress as they work towards recovery, and our therapists are able to adjust their therapy program as needed. Outcomes Report Cards also serve as dynamic marketing tools that you can use to drive census in your facilities, track patient progress and report results.

Historically, this program has been supported with customized equipment from our long-time partner, Nautilus®. However, we are now pleased to also offer a new program with HUR® Health & Fitness equipment that utilizes Natural Transmission technology and the HUR SmartCard. The heart of HUR machines is the SmartCard, used to automate the entire training session, which makes workouts easier and more motivating for all users. With the SmartCard, repetitions and resistance are automatically uploaded. A color touch screen with animated exercise display provides additional motivation and helps beginners ease into the exercise. We use evidence-based practice and physician-specific protocols for diagnosis and treatment. Aegis Therapies has developed the NaviGait program to provide rehabilitation therapy services to patients who have undergone amputation or limb loss and are being fitted with a prosthetic device. NaviGait is designed to ensure that patients receive the clinically appropriate, functional benefits that Aegis has to offer. The program offers primarily physical therapy services and occupational therapy services to patients with limb loss surgery. The initial focus is on lower extremity amputation with pre-operative services (where possible) and post-operative services. The program will expand to include all limb loss patients, as well as additional services, including orthotics, pediatric plagiocephaly and post-mastectomy services.
EVALUATION AND TREATMENT SERVICES INCLUDE (BUT ARE NOT LIMITED TO):
Services include pre-operative visits, when possible and as clinically indicated, as well as post-operative services that will be provided pre- and post-fitting and upon receipt of the first prosthesis. Additionally, a critical component of NaviGait is the use of therapeutic modalities – ultrasound, electrical stimulation and diathermy. These modalities are vital in helping to address issues with pain management, phantom limb sensation, edema management and muscle reeducation. The program also includes balance training. Patient education about predisposing conditions, risk for complications, and prevention strategies is also an important component of the program. Through effective patient education and patient monitoring, we believe the risk of complications and re-hospitalization will be reduced.

DYSPHAGIA DIAGNOSIS
AND MANAGEMENT Aegis Therapies continuously searches for evidence-based methods to improve the health and well-being of patients with a variety of conditions. The needs of patients with dysphagia are currently underserved in the post-acute care industry, and there is an opportunity to greatly improve care for these patients with Aegis Dysphagia Diagnosis & Management. Studies have shown that 78% of all avoidable rehospitalizations that occur within 30 days of admission to a SNF are caused by five specific diagnoses, four of these diagnoses are related to dysphagia. In partnership with Dysphagia Management Systems, Aegis Therapies has developed a program that will provide better care and condition management for these patients. With an aggressive dysphagia management program, we can help reduce unplanned avoidable rehospitalizations by as much as 62% for the following conditions: • Congestive heart failure, which is associated with aspiration, pneumonia and dysphagia • Respiratory infection, which causes or is caused by dysphagia • Urinary tract infection, which is associated with malnutrition and dehydration • E lectrolyte imbalance, which means that the patient with dysphagia should remain nutritionally sound to remain stable Additionally, studies have shown that up to 30% of nursing home residents with dementia are undernourished. Through appropriate endoscopic assessment, it can be determined if the undernourishment is due to an inability to swallow. If so, the dysphagia management program would help the patient improve swallowing ability and therefore reduce problems associated with undernourishment.

HEALTHSTEP There are currently 26 million people with diabetes in the United States. Aegis Therapies is helping to improve the health of this patient population, and is exploring new ways to improve the care and management of these patients in terms of preventing foot ulcers and lower-limb amputations. Individuals with diabetes make up 76% of all lower-limb amputations in the U.S., and approximately half of all diabetes-related amputations have been attributed to poorly fitting footwear (JAMA, 2002). To help address this problem, Aegis Therapies is partnering with Visual Footcare Technologies to introduce Aegis Healthstep, the latest in preventative foot care to people with diabetes.

AEGIS THERAPIES AND VFT Visual Footcare Technologies (VFT) is a hybrid medical device/healthcare services company, which has comprehensive programs and FDA-approved, proprietary products that target foot ulcers and other lower extremity complications associated with diabetes. VFT provides a standardized, comprehensive diabetic foot assessment that uses the company’s proprietary, FDA-approved risk stratification devices. VFT’s Lower Extremity Amputation Prevention (LEAP) program promotes early risk stratification, patient education and proactive prevention of diabetic foot ulcers. VFT also makes available therapeutic footwear and insoles, which are eligible for reimbursement through Medicare’s Therapeutic Shoe Benefit Program. Aegis Therapies will provide trained and certified Therapeutic Shoe Fitters who will understand how to properly assess and fit patients with appropriate footwear. Aegis will help stratify and educate patients who are at risk for diabetic foot ulcers. This will facilitate the initiation of preventative care and treatment, and will positively impact patient outcomes.
WALK!
WITH AEGIS THERAPIES WALK! with Aegis Therapies is a five-day celebration of health and wellness that is held annually each September. It is held at Aegis client locations across the country, including independent living facilities, assisted living facilities, continuing care retirement communities, skilled nursing facilities and other locations. 2015 marks the seventh annual WALK! with Aegis Therapies event. The event is presented by EnerG® by Aegis and the International Council on Active Aging® (ICAA). The event is held in support of Active Aging Week and was launched as a one-day event during Active Aging Week in 2009. The next year, WALK! with Aegis Therapies expanded into a five-day celebration of health and wellness. In 2014, more than 760 locations across the country participated in the program. This past year, the WALK! with Aegis Therapies event was the largest in its six-year history, with more than 61,000 participants at over 760 locations nationwide. Participants walked a combined total of 94,000 miles. Notably, the ICAA has awarded Aegis Therapies an Innovators Award for WALK! with Aegis Therapies. The annual award recognizes creativity and excellence in active aging, and it honors programs, concepts and products that are setting new standards and are making a difference in the lives of older adults. WALK! with Aegis Therapies is a series of daily walking sessions and activities focused on a different dimension of wellness every day, including physical, intellectual, occupational, emotional, environmental, social and spiritual. Throughout the week’s activities, participants are active and hear words of encouragement from Martha Schram, President of Aegis Therapies, and Colin Milner, Founder and CEO of the ICAA, through CDs that are provided to all participating facilities. Each facility has a Director of Rehab (DOR) assigned. The DOR is a manager and is available to meet daily with the facility team based upon the needs of the customer. The DOR is the direct support manager to the therapists and manages the day-to-day business for the therapy programs. The DOR is also a partner with facility management. The Area Vice President (AVP) will be on-site monthly or as requested by the customer. The AVP provides oversight and support to the DOR for the achievement of the goals of facility in partnership with facility. The AVP provides business reviews as well as facility visits as needed. The Regional Vice President (RVP) provides oversight and support to the DOR and AVP. The RVP participates in chain quarterly and yearly business reviews, special strategic initiatives and facility visits as needed.

Aegis is a leader in rehab recruiting. Our recruitment department consists of a team of 16 employees to ensure that needed staffing levels are met. Staffing needs are assessed at the facility level based on patient needs and facility objectives. Aegis Therapies currently employs over 4,400 full-time, regularly scheduled employees. An additional pool of more than 4,000 part-time employees provide backup staff when there are increases in patient caseloads and new business opportunities, and when seven-day coverage is a facility requirement.

University Relationships and New Graduate Recruiting Building and maintaining strong clinical relationships with universities is a major goal at Aegis Therapies. Currently, Aegis has over 2,500 contracts with universities offering clinical rotations and affiliation opportunities across the country. In 2014, a total of 342 FTE new graduates joined Aegis Therapies. In addition, we partnered with Concorde University to launch our own OTA school in response to the lack of these specialized educational institutes available in some areas of the country. We graduated the first class of 7 students in December of 2014 and have plans to expand in 2015. We are doing the right thing to bring in the right people. Conversion and Implementation Plans Aegis Therapies is successful in converting existing staff and hiring additional staff as needed for new business partnerships. When a conversion of existing employees is involved, Aegis is highly competitive in its approach to recruitment. The conversion of contract therapists includes recognition of the number of years of service that the therapists have at the current facility. This is in part due to the thorough benefit comparison, competitive pay and employee meetings that are conducted to ensure a smooth transition for these employees. The benefits and conversion plans are introduced to the therapists prior to start date.
