DISCLAIMER I, ________________________ (Full Name), ___________ (NRIC/Passport No.) hereby consent to participate in the Run to Give 2016 organised by Le Meridien Kota Kinabalu (“Hotels”). I have been informed and understand that there are certain risks associated with Run to Give 2016. To the best of my knowledge, I am in good health and physically willing and able to participate in the Run to Give 2016. I agree to abide by all rules, regulations, policies or procedures established from time to time that the Hotel or any other operator of the Hotel Charity Run 2016 might have governing the operation of the Run to Give 2016 or safety of the participants. I understand that whilst reasonable precautions will be taken by the Hotel to ensure my safety during Run to Give 2016, I am attending the run at my own risk and will be responsible for my own safety. I have read and understand the foregoing consent to participation in the Hotel Charity Run 2016. In addition, I agree to the following: 1. I am at least eighteen (18) years of age or a parent and/or guardian must sign this Disclaimer on my behalf and accept all liability as described herein; 2. I assume all risk of injury and all risk of damage to or loss of personal property arising out of my participation in Run to Give 2016; 3. I unconditionally release, discharge, and waive any and all responsibility of the Hotel, the Hotel’s owner, the Hotel’s operator and Starwood Hotels & Resorts Worldwide, Inc., and all their affiliates, directors, officers, agents and employees, from and against any liability of injury, including death, and for damage to or loss of property which may be suffered by me arising out of, or in any way connected with my participation in the Run to Give 2016; and 4. I unconditionally agree to indemnify and hold harmless the Hotel, the Hotel’s owner, the Hotel’s operator and Starwood Hotels & Resorts Worldwide, Inc., and all their affiliates, directors, officers, agents and employees from and against all liability, claims, demands, actions, loss, and damage arising out of my participation in the Run to Give 2016. I understand and agree that all proceeds collected from the registration fees for the Run to Give 2016 goes towards supporting Bukit Harapan and in the event I do not attend Run to Give 2016, the registration fees that have been paid will not be refunded. The Hotel has the rights to any form of media taken at Run to Give 2016 for any legitimate purposes including commercial advertising and publicity of Run to Give 2016, and I hereby acknowledge and agree that my name, photograph(s) or other reproductions of my physical likeness and/or recordings of my voice may be used by the Hotel in connection therewith. I have been given sufficient time to read this Disclaimer, and fully understand its implications. By signing below, I agree to grant this Disclaimer.
