  As of July 1, 2012, Lifescan OneTouch blood glucose meters and test strips will no longer be on  the Network Health Preferred Drug List and will require prior authorization for patients with no  prior history of Lifescan OneTouch use.  We will continue to cover Lifescan OneTouch blood  glucose meters and test strips until August 31, 2012 for Network Health patients who are  currently using Lifescan products.   As of September 1, 2012, we will require prior authorization  for all patients using Lifescan OneTouch blood glucose meters and test strips.  Abbott products (FreeStyle Lite, FreeStyle Freedom Lite, and Precision Xtra) are our preferred  blood glucose meters and test strips. Network Health patients can get up to two preferred  blood glucose meters per calendar year and 150 test strips per 30 days without prior  authorization.   Not For Retail Sale Products  In addition to these changes, as of July 1, 2012, Network Health will not cover Abbott products  that are designated as not for retail sale.  Network Health will only cover the Abbott meters  and test strips that are intended for retail sale.  The following NDCs will no longer process at  the point‐of‐sale.  Meters  Product Description  FreeStyle Lite System Kit NFRS  FreeStyle Freedom Lite System Kit NFRS  Precision Xtra Meter NFRS
