The University is not responsible for late bids due to off campus mail service. 3. All prices, terms, and conditions must be shown. Additions or conditions not shown on this bid will not be allowed. 4. Prompt payment discounts will not be considered in determining the low bid. 5. Prices quoted shall be less Federal Excise and State Sales taxes. 6. The PSU Director of Purchasing reserves the right to accept or reject any part of this quotation.

OUTDOOR ELECTRONIC MESSAGE CENTER Furnish and Install a replacement Outdoor Electronic Message Center (EMC) at 103 East Cleveland Avenue on the campus of Pittsburg State University per the attached specifications. The scope of work covered by this specification consists of furnishing all labor, equipment, material and proper disposal of materials removed for this project herein unless otherwise noted. Electronic PDF of this RFQ is available at the following web page: http://www.pittstate.edu/office/purchasing/bids.dot .It shall be the bidder’s responsibility to monitor this web page on a regular basis for any changes / addenda. BASE BID (LUMP SUM): For the referenced project and in accordance with specifications for a total lump sum price of:

References: Provide contact information of four (4) references currently using a product of yours that is comparable to Pittsburg State University’s EMC scope. Prebid Meeting: No prebid meeting will be held for this project, however, Vendors are responsible for taking all measurements and ascertaining the extent of work to be completed. Failure to inspect the worksite will not relieve the vendor of the obligation to complete the work as specified by PSU. Contact Chris Kelly at 620-235-6177 to schedule a worksite visit. Insurance: The Successful Bidder shall present an affidavit of Worker's Compensation, Public Liability, and Property Damage Insurance to Chris Kelly, PSU Associate Vice President. Tax Clearance Certification: Bid submittals shall include a copy of a Tax Clearance Certification. Tax Clearances may be obtained from the Kansas Dept. of Revenue (KDOR): http://www.ksrevenue.org/taxclearance.html The PSU Purchasing Office cannot send tax clearance applications on the vendor’s behalf. Please send your application to KDOR. DA-146a Contractual Provisions: The bidder agrees to accept the provisions of form DA-146a, Contractual Provisions Attachment which is incorporated into all contracts with the State http://www.da.ks.gov/purch/DA-146a.pdf W9 Form: Vendors who are new to PSU should submit a copy of their W9 with bid response. Minor deviations in size and operational characteristics from those set forth in the specification will be considered when such deviations do not alter nor deter Pittsburg State University from accomplishing its intended usage or function. Each bidder must clearly indicate in writing where (if any) their product characteristics deviate from these specifications, and explain how their product accomplishes the desired function even though product characteristics may be different. Acceptance or Rejection: PSU reserves the right to accept any or all bids or part of a bid; to waive any informalities or technicalities; clarify any ambiguities in bids; and unless otherwise specified, to accept any item in the bid. Freight Cost Information: Bids shall be submitted FOB Destination, Freight Prepaid, and Allowed. Payment: Payment will be made upon completed installation, successful testing, and acceptance by PSU.

Contractor Installation Responsibilities: • Contact University Police 620-235-4624 a minimum of 24 hours prior to installation. • Removal and disposal of existing equipment. • Provide and install new Outdoor Electronic Message Center (above) in existing brick structure with temperature sensor. • Generate and issue standard product attachment drawings. • Generate and issue site specific equipment shop drawing and attachment detail submittals. • Site clean-up upon completion of installation. • Perform final systems testing and commissioning. • Provide User Training. PSU Responsibilities: • Wired Ethernet to brick structure. • Electrical power to brick structure. Existing power consists of (3) 120V circuits. • Provide computer(s) for control software. • Project acceptance.
