Membership Terms and Conditions What Your Gonna Get In This Relationship. * For an amazing price of $59 per month for your membership you will receive one FREE MASSAGE per month * If you want any more than one massage per month its only another $59! * Massage Addiction memberships are based on a 12 month membership. 12 Month memberships are auto-renewable * Members are required as per this agreement to notify the staff of Massage Addiction, any change of address, credit card / bank account changes and any other information that we require of you. * In your birthday month you will receive ONE FREE add-on (except the add-on time option) * Massage Addiction is not held responsible for lost or stolen items. * Massage Addiction reserves the right to change clinic rules, regulations and pricing at any time upon providing reasonable notice (if requested by the client). * If at any stage of your 12 Month contract it becomes apparent that you are not comfortable within the clinic you can either choose to change therapist or cancel your membership (with a termination fee – see breaking up with us). * You will not lose any free massages, if you cannot make it in for the month it “rolls over” to the next month. The finer details. * You must nominate a bank account or credit card for a monthly fee to be direct debited from. * A monthly payment of cash is not accepted. * If your financial institution declines the direct debit / payment, you will be notified and you will not be able to use the services of Massage Addiction until your membership is brought up to date. Where are you at? * On the day cancellations happen – We live and breathe this everyday. However you must give us 24 hours notice of a cancellation of your massage appointment or you forfeit that one massage for that month. *Your membership allows your partner to take your massage for free. What Do You Mean Your Breaking Up With Us! * If you chose to cancel your membership within the initial 12 month contract period; a termination fee of 50% of the remaining contractual obligation. * IF you wish to cancel your membership at any stage (during the first 12 months or after) you need to submit the request in writing. [email protected]. * Once you have been approved for a cancellation of a membership no more fees shall be ducted from your banking institution. * You may cancel your membership 30 days before the auto-renewable period providing it is in writing, either by email or fax. * You may cancel your membership with no charges if you develop a permanent disability or a chromic disease that will stop you from receiving treatment. * You can cancel your membership if you move more than 50km away from your closest Massage Addiction. Giving Us The Cold Shoulder? Let it Go, Let it Go! * There may be circumstances in which you made need to freeze your membership for and extended time with Massage Addiction. Such as extended illness, holiday, an extended stay at work etc. When you choose to freeze your membership you cannot use it until Massage Addiction staff are told to unfreeze it. * There will be a $5 per month fee involved for a freezing of a membership. * A minimum of one direct debit payment must have been made before you can choose to freeze you membership. * You can choose to freeze your membership on a minimum of 14 days and a maximum of 3 months. At the end of the frozen process the direct debit will automatically start once again. * A membership must be unfrozen for a minimum of 30 days before another freeze may take place. * Your membership / contract date will extend for the amount of time that you freeze your membership for. * Terms and conditions continue to apply within the extend time of your membership. Want Others To Know How Good We Are? * Word of Mouth is the best source of advertising. When you refer a new member, you receive one complementary add-on for every person and it will be available in your next massage treatment. **NB – Cannot upgrade to the half hour or the hour. * These free add-ons do not expire. What we actually do allow. * A child under the age of 16 must be given consent of a treatment by a parent of guardian. * The parent or guardian of the child can either wait in the waiting room or accompany the child into the treatment room. * Massage addiction highly recommends that the child be scheduled with a massage therapist of the same gender; however this is of personal choice. Something For The Ones You Love Or Just An Easy Present. * Massage Addiction gift vouchers are available on request. They are to be paid when purchasing the voucher they cannot be debited from you monthly membership. You can nominate the amount as well as the extras that you want to include in the massage treatm ent. * Instead of paying $85 (a standard massage cost) – You just pay $69.
