I am a player participating in Midwest Coin Concepts Pool League Program and would like to have ranking reviewed. I understand that I must submit this request in writing by completing this form and mailing it to the league office coordinator no later than Dec. 1st. I am aware that my review will be conducted by a committee of selected Midwest Coin Concepts pool players at a date no later than Dec. 31st. Rankings posted Jan. 1 and later will be considered official and cannot be changed without the consent of the Pool Forum Committee and the Tournament/League Director.
