Common Forefoot Issues Arthritis, Bunions, & Hammertoes Over time, our feet take a real beating. As we age, we become more susceptible to developing conditions that can affect these hard-working “dogs.”

ARTHRITIS Arthritis occurs when cartilage – the smooth, gliding tissue, wears away from the joint surface. This can happen as a result of injury, aging, or overuse. Symptoms include pain with motion and at rest, and swelling. When the large toe is arthritic, a bump occurs on the top of the toe. This is not a bunion, however a bump on the inside of the toe joint can also form that creates pain. The pain comes from the grinding of bone on bone surfaces. The toe can get stuck in an unnatural downward position. This toe position makes it difficult to walk or play sports. In extreme cases, people may not want to walk or move at all due to the pain. Once diagnosed, Dr. Kay may initially recommend non-surgical approaches like activity modification, wearing properly fitted shoes, arch supports, or anti-inflammatories like Advil® or Aleve®. Should these options be ineffective, surgery may be considered. There are a variety of options available to manage the arthritic large toe and treatment is always individualized. BUNIONS. A bunion is a bump on the inner side of the big toe. The big toe starts to cross towards the 2nd toe and may go over or under it. Shoes can be very painful to wear because of the toe position. Inheritance plays a strong role in the development of bunions with women more affected than men. Bunion pain is worsened by poorly fitting shoes. The basic treatment is to wear shoes that don’t rub on the toes. A simple at home evaluation is to trace the outline of the foot and place the shoe on top. When the shoe fits, no toes are visible. The only way to properly correct a bunion is with surgery. Surgery realigns the bones and soft tissue support so that they are in the correct position. The number of described bunion procedures is overwhelming and treatment is individualized. Minimal incision surgery, or MIS, may be a consideration. The specific procedure is always individualized. Dr. Kay is the inventor of instruments and corrective implants specifically designed for minimal incision bunion surgery. He is also the inventor of multiple other surgical implants that have been used worldwide. Bunionettes are the “bumps” that form on the outside of the small toe. These can be very painful. The evaluation will include a discussion on non-surgical as well as surgical correction. HAMMERTOE (AKA CLAW TOE). As the name “hammertoe” suggests, the toe(s) become crooked where they’re bent, causing them to rub at the tops of shoes. While hammertoes can be inherited, more often it is the result of the small muscles in the foot becoming overpowered by the larger muscles. It can also be caused by wearing poorly fitting shoes. Once diagnosed, Dr. Kay may suggest wearing small splints to pull the toes downward, wearing shoes that don’t allow the toes to rub, or using pads on the toes for protection.

Should these approaches fail, surgery may be necessary to adjust the ligaments and tendons so the toes straighten. Traditionally small wires or pins are left protruding from the ends of the toes for up to six weeks. These are removed in the office setting. However, there are now small fusion implants that have greatly reduced the need for protruding pins. Cardiovascular Disease & Men: It’s Time to Look at Testosterone

By Tara D. Scott, MD, FACOG, FAAFM, ABOIM, CNMP When you think of the hormone testosterone your mind probably wanders to the bedroom, and you would be correct! In addition to testosterone’s role in libido and sexual function, it can also affect energy levels, muscle mass, motivation, and mental clarity. One of the lesser known benefits of testosterone is its positive impact on cardiovascular system. There use to be a belief that high testosterone levels can increase your cardiovascular risk, but studies done over the past 20 years have shown that this may not be the case. We now think that low levels of testosterone are associated with an increase in abdominal fat, insulin resistance, inflammatory markers, low HDL and high triglycerides, all markers of cardiovascular disease. A study from the European Heart Journal showed that men with coronary artery disease (CAD) had lower levels of testosterone and testosterone markers as compared to men without CAD, while another study in the same journal showed low testosterone markers were associated with an increased risk of premature CAD. Men’s testosterone levels start to naturally decline in their late twenties early thirties. Some men are able to maintain optimal levels

while others may need a bit of help. Although this decline is natural, there are certainly other factors that may contribute to it: poor diet, inadequate exercise, stress and more. So it is just as important for men to know their hormone levels if they are not experiencing optimal health! The Elephant in the Room (Let's Talk about Mesh!) By Mary South, MD The only way to adequately address the topic of transvaginal mesh is to face it head-on. I know there are patients out there who have had a midurethral sling* with mesh that resulted in complications. Those individuals may wonder why, if we (the surgeons) are aware of those issues, would we continue to offer and perform this procedure for women with stress incontinence? The answer is that the midurethral sling, in the big scheme of things, results in far more benefit to women compared to the risk of complications. I would never deny that there can be complications from this procedure. However, complications can occur with any surgical (or medical) procedure. As a surgeon who performs this procedure, the overwhelming majority (more than 95% of patients) are satisfied with the results of their procedure. I tell my patients I would recommend a sling for stress incontinence to my mother, my sister, my daughter and my friends.

As a matter of fact, one of my friends did come to me for a sling about three years ago, and I asked her to write down her experience. “After giving birth to my two children, I developed stress incontinence when I sneezed and coughed. As a healthy, fit 37-year-old, I did not want to settle for dealing with this issue for the rest of my life. I made an appointment with Dr. South, and she suggested a mesh transvaginal sling surgery. I can jog, jump rope, and box jump with NO issues. It was simply an amazing process, and I would recommend it to anyone dealing with stress incontinence issues.” *I am specifically referring to transvaginal mesh in the form of a midurethral sling for stress incontinence. I am not referring to transvaginal mesh for prolapse repairs.

Estate Planning: Wills may not always be the best way to transfer property By William Muniak, Attorney at Law Many believe that a Last Will and Testament is the “magic bullet” of estate planning and that a Will assures an immediate and inexpensive transfer of property to loved ones. Not true! While a Will is certainly an essential estate planning document, it is often an inefficient means of transferring property to others. Here’s why: A Will is a safety net which is needed to dispose of property that is not gifted prior to death or properly designated to transfer to others upon one’s death without going through probate. Indeed, the use of a Will means that some property remains in the decedent’s name after death and that the remaining property will have to be "probated" in what can be a costly, lengthy and public court proceeding. While there are certainly valid reasons to utilize a Will to transfer property, there are also efficient alternatives (many of which do not require a lawyer). For example, individuals and/or couples can transfer the family home and/or other real property to children through the use of a Transfer on Death Designation Affidavit. This Affidavit, if properly drafted and executed, can be recorded with the County Recorder during one’s lifetime, resulting in the prompt transfer of that same property to others upon one’s death (after the filing of a death

certificate) without a court proceeding. Similarly, one can add Payable on Death or Transfer on Death designations to other property (bank accounts, annuities, stocks, etc.) in order to quickly transfer those assets to others upon one’s death. With some simple planning, even a millionaire can pass without a dime of probate property, and that is not necessarily a bad thing. It is not fatalistic to consider the inevitable. Plan on it!

Learn to Downsize Before a Move Aging men and women often take inventory of their lives to focus on activities or lifestyle changes that can ensure happy retirements. Data from the U.S. Census Bureau shows there are some 76 million baby boomers. With the youngest of the boomers in their mid-50s and the majority having already reached retirement age, many boomers are trying to decide if it's time to move out of their family homes and into smaller, more manageable abodes. Many older adults find they do not need the same amount of space. Retirees and those on the cusp of retirement may find that downsizing is a smart financial move and frees up time for recreation. However, it can be challenging to cut down on living space and then deal with figuring out how to make furniture, belongings, and stored items fit in more condensed areas. Moving can be stressful even without having to cut down on prized items. Before moving, men and women can go room by room, making piles of items that will be kept, donated, sold, or discarded. It can be a tedious task, but it is necessary to avoid clutter in a new home. People downsizing can attempt to sell items they do not need. Any extra cash can be put toward buying new items that are sizeappropriate for the smaller home. Another way to clear out clutter is to sort duplicates from the 12

stock of items. A person may no longer need multiple sets of dishes or silverware. If the move involves switching from a kingsized to a queen-sized bed, donate or trash bed linens that will no longer fit. Pay close attention to kitchen and bathroom items, which tend to accumulate over time but might not be discarded when clearing a home of clutter. People moving from a detached home to a condominium or a townhouse may learn that homeowner's association fees cover everything from snow removal to lawn maintenance to pool upkeep. If so, it's unnecessary to bring lawn and garden supplies. Homeowners are advised to look at the floor plan of their new dwellings and pay attention to storage space. This can make it easier to plan ahead for what may fit, what to purchased new and which storage solutions may be needed. Having a plan in place can make unpacking and settling in go smoothly. The organizing company Organize Me says that homeowners should consider how cabinets and closets will be used before moving in. Downsizing can free up time and money. When done right, downsizing can make retirement easier and create more leisure time for retirees.

Keep Your House Running Smoothly this Summer! The day-in, day-out work of running a home can be overwhelming. Add to that the extra demands of summer "break" and it's a lot. Here are a few suggestions to keep your house running smoothly this summer! 1. FLOORS. When in doubt, sweep or better yet vacuum. Summer means lots of in and out of the house for you, your kids and your pets! I can handle a certain amount of disarray, but when I'm starting to feel twitchy, I know it's time to break out a broom or vacuum and attack the floors. 2. PUT AWAY THE EXTRAS. All you want to do when you get home from vacation, the pool, the beach or that baseball game is relax, but take a few minutes and have everyone pitch in and put everything away, the correct way. Gather all the dirty clothes and take them to the laundry room; put away all the clean clothes and toiletries and return the suitcases to storage. It feels like this is going to take forever and that it might kill you, I know, but doing it will make you feel great! Do you go to the pool every day in the summer or a few times a

week? Another tip: when you clean your pool/beach towels and swimsuits put them right back in your "pool" bag so you will have everything ready to go! Save on time and space. 3. Should I Get Remarried? Part II

Estate Planning Issues Surrounding Remarriage

Salek, Attorney Certified Elder Law Attorney (CELA) by National Elder Law Foundation & OSBA A second marriage later in life can trigger several issues regarding who will receive assets upon one spouse's death. However, additional problems may arise related to a spouse needing long term care (nursing home/assisted living). As people age, their mental capacity may diminish due to dementia, Parkinson's, Alzheimer's, stroke, etcetera. In such a scenario, the person should have certain Powers of Attorney in place. Issues often arise when determining who should be that Power of Attorney. Should a relatively new spouse serve in that role or should a child from a previous marriage serve? If a court guardianship needs to be implemented, even more complicated issues arise regarding who should serve. When determining how to pay for long term care, a patient, as well as their spouse, may become liable for the cost of long term care. This is the case even if a pre-nuptial agreement states that each spouse will keep their assets separate. For example, in determining whether a patient qualifies for Medicaid benefits to assist in paying for long term care, both spouses' assets are counted. This is true regardless of whether or not the spouses agreed to keep assets separate or both agreed to be liable

for their own cost of care. Without specific planning otherwise, assets of one spouse become susceptible to being spent for the long term care of the other spouse. On a more positive note, marriage to a veteran may allow the spouse of a veteran to qualify for certain benefits in the future. The widow of a veteran may qualify for benefits called Aid and Attendance, which may assist in the cost of long term care. In such a case, the remarriage may be a significant benefit to a future widow. While remarriage can trigger many significant issues, most can be overcome with proper planning. A second marriage can be a wonderful time in life. How to Choose a Dementia Caregiver When a loved one is diagnosed with dementia, a family's life can turn upside down. In such situations, families may not know much about the disease, including what to expect with treatment and how soon before the dementia patient begins to need care that the family cannot capably provide. Over time, dementia patients' loved ones are likely to benefit from the expertise and assistance of qualified dementia caregivers. It can be overwhelming for loved ones to offer the right level of care for someone unable to perform the activities of daily living. Bathing, medication management, dressing, and feeding are often very difficult for dementia patients. The Alzheimer's Association says that providing proper care for someone with dementia goes beyond meeting basic needs. To get started, one should first assess the needs of their loved one with dementia. How many services he or she will require depends on whether that person can use the bathroom, walk, eat, or bathe independently. Alz.org says care needs tend to be lesser in the early stages of dementia. However, during the middle and end stages of dementia, 24-hour supervision, and potentially more intensive medical care may be necessary. Some families start with a visiting caregiver who can come to 16

the house. For example, a service like Visiting Angels is certified to offer care according to advanced dementia care protocols after working with leading dementia specialists. Caregivers may provide companionship and helpful reminders. Others may assist clients with personal tasks. One key aspect of dementia care is preventing wandering. Alz.org indicates that six in 10 people with dementia will wander. A person with Alzheimer's may not remember his or her name or address and can become disoriented, even in familiar places. Caregivers can put protocols in place to help reduce wandering. Alert bracelets and GPS tracking devices can help in this regard as well. At some point, caregivers can help families transition someone with dementia to nursing facilities with memory care divisions. Social workers and other aides may help families navigate the legalities of medical insurance and long-term care insurance as well as government assistance programs that may help offset the costs of more intensive care. It's never too soon to develop a care plan for someone with dementia. Qualified and compassionate caregivers can help ease the burden of dementia on patients and their families.

1. Put a “Before Screen Time” list up to help encourage learning activities while you are gone. 2. Set up a buddy system with other parents and take turns planning an activity. 3. In 2006, my family started Lincoln Way Vineyards, but it wasn’t until October 2017 that we Found Our Wine Time making our first batch of wine professionally. Our children, ages 15 and 17, have grown up with the vines and become an integral part of the family operation. Alex operates and maintains equipment as well as oversees harvest and crush. Arianna mows and is involved in the winemaking process running lab analyses on the grapes, must and wine. Jim, my husband, directs it all while taking care of our social media, festivals, and tasting room. His family background in farming and his career in water/wastewater treatment led to his interest in raising grapes and making wine. My focus is primarily on marketing, accounting, blending wine, and quality control. As a brand manager for a boutique wine distributor and as an accounting and inventory controller for a national scrap processor, this gave us the perfect combination of experience to want to dive into this endeavor. Why should your child learn to swim? There are many reasons why a child should learn to swim. But the most important reason is that knowing how to swim is the only activity that can save your child’s life. According to the Centers for Disease Control and Prevention, approximately two children ages 14 and younger die each day from accidental drowning — and it’s the No. 1 cause of death for kids ages 1-4. It is absolutely crucial that all kids know how to swim at a young age. There is water all around us, even if it’s as small as a bathtub. Making sure that your child is comfortable in and around water is essential to their safety. And with the summer months upon us, the opportunities for drowning increase. Swimming is lots of fun for people of all ages and children especially love getting in the water and enjoying themselves. But it is not just fun, swimming also provides loads of health benefits which can help to keep your children healthy and happy at the same time: • Swimming keeps your child’s heart and lungs healthy, improves strength and flexibility, increases stamina and even improves balance and posture Another great thing about swimming is that children of any age or ability can take part and it is more accessible for children with additional needs than almost any other sport. •.Swimming provides challenges and rewards accomplishments, which helps children to become self-confident and believe in their abilities. • Your child will have plenty of opportunities to make friends and grow in confidence But health, fun, and confidence are not the only reasons why your child should

MAKING A DIFFERENCE

Personally, and as Caregivers, Women More Often Witness the Devastation of Alzheimer’s According to the Alzheimer's Association 2019 Alzheimer's Disease Facts and Figures, 5.8 million Americans are living with Alzheimer’s or another form of dementia, including 220,000 in Ohio, figures that grow year after year as the baby boomer generation ages. Alzheimer's is a type of dementia that causes problems with memory, thinking and behavior. Symptoms usually develop slowly and get worse over time, becoming severe enough to interfere with daily tasks. Dementia is often incorrectly referred to as "senility," reflecting the formerly widespread but incorrect belief that serious mental decline is a normal part of aging. Many do not realize Alzheimer’s is a disease that predominantly impacts women: almost two-thirds of Americans with Alzheimer’s are women. Women in their 60s are more than twice as likely to develop Alzheimer’s over the rest of their lives as they are to develop breast cancer. Alzheimer’s, the sixth leading cause of death in the U.S., remains the only one of the top ten leading causes of death with no method of prevention, treatment, or slowing it down. Not only are women more likely to have Alzheimer's, they are more likely to be caregivers of those living with Alzheimer's. • More than three in five unpaid Alzheimer's caregivers are women. • More than one-third of dementia caregivers are daughters. • Because of caregiving duties, women are likely to experience adverse

consequences in the workplace. Nearly 19% of women Alzheimer's caregivers had to quit work as a result of caregiving responsibilities. Women caregivers may experience higher levels of depression and impaired health than their male counterparts. Evidence suggests these differences arise because female caregivers tend to spend more time caregiving. During Alzheimer’s & Brain Awareness Month in June, the Alzheimer’s Association raises awareness of the importance of early detection and diagnosis and the free local programs available to lessen the burden on caregivers, programs such as caregiver support groups, early stage engagement programs, education presentations for clubs and organizations, one-on-one care guidance from chapter social workers, and a 24/7 Helpline: 800.272.3900. Signature events like The Longest Day® in June and the Walk to End Alzheimer’s®, held annually in more than 600 communities nationwide in the fall, allow local chapters to provide services for free while supporting cutting edge research.
