Dear Sir RE: LISMORE BASE HOSPITAL HLS VFR APPROACH AND DEPARTURE PATH SURVEY REPORT.

Section 1 (Approach/Departure and Transitional Surface Obstacles) An obstacle survey for the Lismore Base Hospital HLS has been conducted on the VFR approach and departure paths and transitional surfaces. The Northern approach/departure bearings are 150°08’26”/330°08’26” respectively. The Southern approach/departure bearings are 0°08’26”/180°08’26” respectively. These bearings are orientated to the Map Grid of Australia (MGA) GDA94, Zone 56. The approach/departure gradients extend upwards at 2.5° for 3500m from the HLS as documented in section 2 of this report. The survey has been carried out to determine if any obstructions penetrate the VFR approach/departure paths and transitional surfaces. At the date of the final survey being the 9th June 2016 the building construction was complete. The only obstructions found penetrating the flight path are the lift overrun and associated infrastructure attached to the lift overrun. The objects that encroach into the VFR are detailed on sheet 9 of the accompanying plans No obstructions penetrate the Northern VFR approach/departure paths or transitional surfaces beyond Chainage 2.38 from the FATO (Chainage 14.855 from the HLS centre) being the northern corner of the lift overrun. This point is referred to as point 1 in the table on Sheets 9. No obstructions penetrate the Southern VFR approach/departure paths or transitional surfaces. The accompanying plans and profile drawings have been provided to show the relationship between the VFR Approach and Departure Path and the existing ground features. The Chainages denoted ‘CH’ on sheets 1 to 8 indicate the distance along the flight path centreline from the FATO and are measured in metres. The Chainages shown on sheet 9 indicate the distance along the flight path centreline from the HLS centre and are measured in metres. This convention has been adopted to eliminate negative chainages on sheet 9. Section 2 (HLS Details and VFR-Transitional Surface Geometry) The flight path geometry in based generally on the NSW Ministry of Health Hospital HLS Policy Guidelines Rev 07. The flight paths commence at the forward edge of the Final Approach and Take Off area (FATO) from eye height (1.5m above the HLS surface) and extends out to 3500m at a gradient of +2.5°. The Transitional Surfaces (wings) have a gradient of 1V:2H sloping up from the VFR approach/departure paths. The transitional surfaces grade upwards from the VFR approach/departure paths to an alignment 75m offset from the flight path centreline. The As built location for the HLS centre at Lismore Base Hospital is: Latitude: 028°48’32.63842” South WGS84 (ITRF at Date Epoch 2016) Longitude: 153°17’31.71335” East WGS84 (ITRF at Date Epoch 2016) Latitude: 028°48'32.67619” South GDA94 Longitude: 153°17'31.69587” East GDA94 Easting: 528506.393 MGA94 Zone 56 Northing: 6813131.726 MGA94 Zone 56 Elevation: 73.9m A.H.D. Diameter: 25m The (MGA94 Zone 56) approach and departure bearings are: North: 150°08’26”/330°08’26” respectively South: 0°08’26”/180°08’26” respectively The TRUE NORTH approach and departure bearings are: North: 150°00’00”/330°00’00” respectively South: 0°00’00”/180°00’00” respectively Section 3 (Survey Datum’s and Background metadata) th

The HLS coordinate and AHD RL have been determined by field survey on the 9 June 2016. Approach and departure bearings have been calculated by applying the average grid convergence of the nearest state survey marks being PM8029 and PM 8037 to the true north directions shown on Figure 20 and figure 21 on page 44 of the Avipro aviation report dated 22nd December 2014. The average grid convergence correction applied is 0°08’26”. State Survey Mark PM8029 has been adopted as the AHD origin. The scan / cloud data was recorded by a Riegl VZ-1000 laser scanner between the 17th November 2015 and the 19th November 2015. Critical points and survey validity checks were measured on the 19th November 2015 using a Leica TCRP1205 and Leica GNSS. Scan positions have been observed using the Allday RTK network. RTK corrections were streamed from the continually operating references station in Lismore. Scan positions have been scaled to ground around the HLS centre by applying a scale factor of 1.000401828. rd

The aerial photography was supplied by LPI on 3 December 2015. Imagery has been scaled to ground based on observed (scanned) ground features at the northern and southern ends of the flight path. No orientation or rotation adjustments have been made to the LPI imagery. th

The scan / cloud data was recorded by a Riegl VZ-1000 laser scanner between the 17 November 2015 and the 19th November 2015. This scan data has been registered to the scaled MGA ground scaled coordinates referred to above. Critical points and survey validity checks were measured on the 19th November 2015 using a Leica TCRP1205 and Leica GNSS. ORANGE •
