You should provide information about when it was established, numbers of patients treated, what treatments you offer (including specific technologies) and your practice ethos. Describe quantitative successes in the last 12 months including % achievement of targets e.g patient retention and financial (250 words) 4. How does your clinic ensure that high levels of clinical expertise and patient safety are maintained? Please provide details of the certification maintained by the clinic, the name and training of the clinic's Medical Director (250 words) 5. Give specific information regarding protocols for assessment and post-procedure follow up as well as handling complications and adverse events. (250 words) 6. Describe your marketing initiatives in the last 12 months and their outcomes (250 words) 7. Tell us about your team and its structure. What performance measurement tools for success do you use and how are these managed by senior staff members? (250 words) 8. How has the team developed and what training have they undergone in the last year? (250 words) 9. How do you collect and monitor customer feedback and how do analyse and respond to the results?
