Unlimited Group Classes Contract
Client Name Please Print Name and Total Health & Fitness Inc., DBA Finally Fit Studio both desire to enter into this contract for the Unlimited Class use of the specified classes below. The Unlimited Class use solely includes: 1. Group Reformer Pilates Classes 2. Group Circuit Pilates Classes 3. Group TRX® Classes Contract Term and Automatic Renewal: Contract is in force from the date Client enters into and signs the Contract (Contract Start Date), with automatic renewal for subsequent months, unless cancelled by either Client or Finally Fit Studio in writing. Cancellation must be received by Finally Fit Studio at least Thirty (30) days in advance of the next Monthly Billing Date. Email is an acceptable form of cancellation in writing to: Email Address -
[email protected]. Payment: Client agrees to pay Finally Fit Studio the below monthly recurring fees. All fees due will be charged each month that the contract is active. If the Monthly Billing Date falls on a weekend, holiday or a day when Finally Fit Studio is closed, or credit card system is down, then the Monthly Billing Date shall be the next calendar date available. Actual client credit card may be charged up to Seventy-Two (72) hours from the Monthly Billing Date. Monthly Recurring Fee: $225.00 fee per month Contract Hold. Clients will be able to put their contract on hold for a maximum of Two (2) months out of each calendar year, and Two (2) times per year from their Monthly Billing Date. The hold may be for Two (2) consecutive months if needed. A Ten dollar ($10.00) fee per hold request will be charged. Any Client may put their contract on hold due to health reasons for up to Six (6) months without any charge being assessed. A written notice from a doctor is required for a medical hold as well as to reactivate their contract stating from Client’s doctor that they are cleared to participate in Finally Fit Studio programs. After Six (6) months, medical hold contracts will be cancelled. Reactivation Fee. If a Client chooses to cancel their contract by giving proper notice, and then wishes to reactivate their contract, a Twenty-Five dollar ($25.00) fee will be charged. Finally Fit Studio reserves the right to waive this fee at their sole discretion.

Additional Terms. 1. I agree & understand that payment for the Unlimited Monthly Classes will be an automatic charge to my credit card each month by Finally Fit Studio on the date (Monthly Billing Date) I entered into the contract, 2. I hereby certify that I am the holder of the credit card and authorize Finally Fit Studio to charge my credit card monthly, 3. I understand that I will be notified if my credit card fails to authorize for any reason, and that a Ten Dollar ($10) late fee may apply, 4. I understand that my service will be deactivated if my account becomes more than Thirty (30) calendar days late, 5. I understand that if I do not show, and do not cancel (according to Finally Fit Studio Policies) for a class I am scheduled for, my credit card will be charged a Twenty Dollar ($20.00) NO SHOW FEE, & a Ten Dollar ($10.00) LATE CANCELATION FEE, 6. To cancel this contract, or change to a different contract, I must provide written notice Thirty (30) days prior to my next Monthly Billing Date. Email is an acceptable form of written notice, 7. I understand that all Studio Policies apply and that Finally Fit Studio reserves the right to increase the fees no more than once every Six (6) month period, 8. This contract is non-transferable and is only for use by the Client named in the contract.
