Who We Are People Inc. was founded in 1970 when a small group of parents and professionals joined together to address the needs of individuals in our community with intellectual disabilities. Since then, People Inc. has remained steadfastly committed to helping people with developmental disabilities and special needs lead healthy, independent and productive lives. Today, People Inc. has grown to support more than 12,500 people in Western New York and Rochester. Our more than 3,800 employees are working in nearly 200 locations so those we serve can move closer to their life goals and dreams. People Inc. offers a variety of services including residential, employment, community outreach, health care and senior services. We are the largest agency of its kind in Western New York and are committed to providing person-centered services and empowering the people we serve. We are committed to operating in the public interest on behalf of people with special needs and their families.

Mission, Vision and Values MISSION People Inc. exists so that individuals with disabling conditions or other special needs have the supports they need to participate and succeed in an accepting society. Our Partners Over the years, we have collaborated, affiliated, established or merged with a number of organizations to create our People Inc. family of partners.

People Inc.’s Elmwood Health Center offers a range of primary and specialty health care services to individuals with disabilities, children, seniors and the general community. Headway of Western New York was founded to support and advocate for persons with brain injuries and other disabilities and is the Regional Resource Center/Single Point of Entry for the Department of Health’s Nursing Home Transition and Diversion Waiver and the Traumatic Brain Injury Waiver. Certified, Licensed and Consumer Directed Home Health Care was established for those with short-term health needs which can be cared for by visiting nurses, therapists or home health aides. Rivershore, a division of People Inc., was founded in 1980 to support people who live with disabilities as they pursue and achieve a meaningful life. Services focused in Niagara County. -5-

Southeast Works, a division of People Inc., was founded in 1974 to help people with developmental disabilities lead productive, independent and fulfilling lives. This division has a focus on employment opportunities. A project of People Inc., the Museum of disABILITY History is the only one of its kind in the U.S. and is dedicated to advancing the understanding, acceptance and independence of people with disabilities. The Museum’s exhibits, collections, publications, archives and educational programs create awareness and a platform for dialogue and discovery. PFM, a durable medical equipment provider, was established to enable individuals with disabling conditions to reach their functional potential in their homes and communities.

Our Approach When it comes to supporting people with developmental disabilities and special needs, each person goes through Admissions to determine eligibility or funding/registration, and then a service coordinator will assist the individual with accessing the services they want and need. A service coordinator’s role is to help people achieve their unique goals and desires. By developing a tailored Individualized Service Plan (ISP), they work with the individual and his/her support team on a semiannual basis to reevaluate what programs and services are most fitting and beneficial.

Individuals we serve often receive an Individualized Service Plan to ensure they can get the most out of life, secure meaningful employment and become accepted participant in society.

Support Team Our service coordinators are a front line of support and main advocates for the individuals we serve and their families. Service coordinators are the most well-versed when it comes to an individual’s goals, dreams, desires and daily living activities. It is their responsibility to maintain Individualized Service Plans (ISP), which outline valued outcomes and aspirations, and detail the supports and services needed to achieve them. Service coordinators are also there to protect the rights of the individual and voice any concerns they or their loved ones may have. As the closest to your loved one, service coordinators are the best resource for questions or concerns. Service

coordinators are backed by a support team of others who work behind the scenes for those we serve. Service coordinators and other staff work in partnership with you and your loved one. Responsibilities of parents/guardians remain the same. Our services are designed to support you and your loved one in living as fully and independently as possible.

Educational Advocacy Age: 18-26 Helps families and service coordinators navigate within the Special Education system through counseling and assistance. Personal Enrichment Program (PEP) Supplemental day habilitation program for individuals with developmental disabilities who do not live in a certified setting. Age: 16-23

Community Active People Program (CAPP) Participants work on personal goals to help increase independence. Age: 18+ Day Habilitation Assists individuals with becoming participating members of their community. Each location has different offerings and allows participants the opportunity to foster community integration through volunteer experiences, career exploration, socialization and daily living activities. Senior Day Habilitation A semi-retirement program for seniors with developmental disabilities, offering a comfortable and flexible environment for participation in creative and social activities. Age: varies Young Adult Life Transitions (YALT) Program Community pre-vocational program that offers young adults an opportunity to experience life on a college campus and prepare for future employment opportunities Age: 18-26

Day Habilitation Without Walls Allows individuals to work with staff and develop a program based on their interests and needs using community resources and facilities. Age: 18+

FAMILY SUPPORT SERVICES Community Behavioral Health Unique, hands-on, in-home support program that works with families of children and adults with special needs who exhibit maladaptive behaviors. Age: varies Community Habilitation Allows individuals to work 1:1 with staff in their homes and community to assist them with gaining independence, productivity and inclusion. Age: 16+ Environmental Modifications/ Adaptive Technologies/Access to Home Adaptation to the home to increase or maintain a person’s ability to live in his or her own home. Age: all Family Education and Training Enhances the decision-making capacity for the family unit by providing an orientation regarding the nature and impact of developmental disabilities, detailing service alternatives and assisting families with understanding and coping with developmental disabilities. Age: all Family Reimbursement An income-based program that assists families with providing various goods and services to their loved ones currently living at home. Age: all Guardianship Designed to assist families with life planning. The program ensures that an individual with a developmental disability is provided for when the current guardian is no longer able to act on his or her behalf. Age: all - 19 -

Life Quality Coaching Provides an opportunity for individuals to work towards a goal that doesn’t necessarily fit into the box of traditional programming. All goals are short term and individualized for each participant to help them foster independence and create a greater quality of life. Age: all Personal Enrichment Program (PEP) Supplemental day habilitation program for individuals with developmental disabilities who do not live in a certified setting. Age: 16-23 Pooled Trust Program – WNY Coalition Pooled Trust Provides options when planning for financial needs. A supplemental needs trust enables a love one’s quality of life to be enhanced, without causing the loss of government benefits. Age: all Self Directed Services A person-centered program to empower individuals with developmental disabilities and their families to design and manage a plan based on their needs and preferences to enjoy meaningful relationships, experience personal growth, live in the home and community they choose, work, volunteer and enjoy recreational activities. Options include skill-building and respite/recreational activities, as well as assistance with employment opportunities. Age: varies Skills Enhancement Program Develops and enhances relationship and communication skills, beneficial to young adult and their families. Age: 15-24

HEALTH SERVICES Adolescent Rehabilitative Services Services that help to optimize the independence of adolescents at home, at school and in the community through management of physical and psycho-social needs. Age: 10+ Certified and Licensed Home Health Care In-home nursing, physical, occupational and speech therapy for individuals with a developmental disability and their household members. Age: under 18 Individual Residential Alternative (IRA) Designed to accommodate the individual’s unique residential needs. Staff is specially trained to encourage the highest level of independence and decision-making through 24-hour care (or less if needed). Age: varies

Intermediate Care Facilities (ICF) Residential setting for individuals with a developmental disability who have selfcare needs and more intensive medical and/or physical requirements. Age: 18+ Perfect Fit Program Living situation with state and agency support that provides a safe, family-like environment and integrated community involvement matched to the individual’s interests. Supervised and Supportive Apartments Independent residential setting for Individualized Service Environment (ISE) individuals with a developmental Self-directed, individualized support to disability. Supervision and guidance is based on the resident’s individual needs. individuals living on their own, either alone or with roommates. Age: 18+ Age: varies

Payne Avenue Respite Located in North Tonawanda, a respite house for children and young adults with a developmental disability who are medically frail and require medical attention. Age: 10-21 Respite Houses The Downing Family Respite Center in Lancaster and the Northtown Respite House in Wheatfield offer planned and emergency on-site care, by the hour or overnight, for individuals with a developmental disability. Age: 10+ Weekend Opportunities Friday and Saturday evening program for individuals with developmental disabilities. Locations in Tonawanda and Orchard Park. Age: 10+

VOCATIONAL SERVICES PROMISE To support schools and communities in providing high quality transition services for Supplemental Security Income (SSI) youth. Age: varies

Business Enterprises Businesses operated within People Inc. to provide employment opportunities for those we serve, including janitorial services, vehicle detailing and more. Age: 18+ Employment and Training Program (ETP) Offers job seekers opportunities to pursue internships in community businesses with the goal of gaining paid employment. Age: varies Job Placement Program Assists individuals with disabilities with employment in the competitive work world. This service is designed to provide admission/initial assessment, job development, job placement and intensive on-site training and related support services on a short term basis. Age: 18+

Supported Employment Program Helps individuals with developmental disabilities with assessment of employment interest and choice, job development, job training, stabilization and long term follow along. Age: varies SaVE Transition (School to Work) Prepares students and their families for what resources are available in the community to support each student as they transition from school to adult life. TRAUMATIC BRAIN INJURY SERVICES Education and Advocacy Services A Speaker’s Bureau is available for talks with schools, community agencies and other groups on topics related to brain injury, awareness and prevention. Nursing Home Transition and Diversion Waiver Services Offers Medicaid eligible persons with disabilities and seniors, ages 18+, a way to live in the community and avoid nursing home placement or to leave a nursing home. An applicant must be Nursing Home Eligible. Some services have specific criteria. Service eligibilities: Medicaid and nursing home residents with disabilities, ages 18+ and seniors 62+ Support Referrals and Resources Assists with guiding individuals to professional services, facilities and programs, particularly those with brain injury and cognitive disorders. Service eligibilities: individuals with brain injuries or related cognitive disorders and their families

Traumatic Brain Injury Waiver Services Offers Medicaid eligible persons with documented brain injury, ages 18 to 64, additional supports to live in the community and avoid nursing home placement or to leave a nursing home and return to the community. An applicant must be Nursing Home Eligible. Some services have specific eligibility criteria. Service eligibilities: Medicaid and Nursing Home eligible persons with documented brain injury, ages 18 to 64 Veterans Services Provide information regarding veteran services particularly with those who have brain injury in the WNY area. Service eligibilities: veterans and their families

Support Groups Groups meet regularly to provide support for survivors of brain injury, including groups for peers, and caregivers. Service eligibilities: Brain Injury, related cognitive disorders and their families

SENIOR SERVICES People Inc., Center for Elder Law and Justice and the New York State Long Term Care Ombudsman Program Provides advocacy and resources for residents who live in nursing homes, assisted living and licensed family type homes in Erie, Niagara, Chautauqua and Cattaraugus Counties. Volunteer opportunities are available for those who work in professional fields and for recent retirees. Age: varies Senior Companion Program Allows adults, age 55+ in Erie County who meet income requirements, to remain independent in their homes. Volunteers receive a stipend and are paired with participating seniors. Age: 55+

Senior Outreach Connects adults who live at home with the services they need to continue independently living as they age. The program is available to seniors who live in Kenmore, Grand Island, North Buffalo, Tonawanda (city and town) and West Buffalo. Age: 60+ Seniors Unlimited Adult Day Care Social day program for adults with dementia or memory loss caused by Alzheimer’s or Parkinson’s disease, stroke, heart disease or other illnesses. The program benefits the whole family, providing caregivers respite as participants receive socialization and activities in a safe environment. Age: varies

Senior Living Apartments Safe and affordable housing for independent seniors who are income eligible. Apartments consist of a bedroom, kitchen, bathroom and a living/dining area. Our Commitment to Quality People Inc. is committed to continuous improvement, and our Quality Improvement Department is dedicated to the evaluation and ongoing advancement of our programs. People Inc. not only follows standards of quality as dictated by The Council on Quality and Leadership (CQL), but also monitors quality through our extensive Quality Improvement Department. We take a multi-layered approach to ensure the quality and progress of our programs. The Quality Improvement Department at People Inc. consists of three teams that work together to ensure a high standard of service. INTERNAL REVIEW TEAM The Internal Review team conducts record reviews, observations, physical plant, fire safety checks and satisfaction surveys for all Office for People with Developmental Disability (OPWDD) programs. INDIVIDUALIZED RESIDENTIAL ALTERNATIVE (IRA) QUALITY SUPPORT TEAM IRAs are residential settings designed to accommodate individuals’ unique needs and encourage the highest level of independence and decision making through 24-hour care. The IRA Quality Support team works with our IRA programs to provide follow up and needed training to assist managers in meeting improvements. INCIDENT MANAGEMENT The Incident Management team is dedicated to thorough and objective reviews of incidents to objectively find the facts of a situation with recommendations to prevent future situations and provide corrective action for what occurred. The team is comprised of full-time staff members with no relation to a program who investigate events - 28 -

that could impact a person’s well-being or quality of life.

Person-Centered Excellence Accreditation from The Council on Quality and Leadership (CQL) In recognition of our commitment to person-centered excellence, People Inc. was awarded the coveted four-year Person-Centered Excellence National Accreditation from The Council on Quality and Leadership (CQL). For more than 40 years, CQL has been a leader in raising the bar for human service organizations and systems to continuously define, measure and improve the quality of life of all people. Organizations that are awarded accreditation by CQL meet a stringent set of criteria and demonstrate an on-going commitment to personcentered excellence. We are the first agency headquartered in Western New York to achieve this accreditation, which confirms that we share CQL’s vision for person-centered solutions to improve the quality of life for people receiving supports and services. As part of this accreditation, we will continue to enhance our systems directly related to person-centered services and in empowering the people who receive support from us to make choices in how they live their lives … where they work, where they live and how they create a community. In addition, People Inc. is now using Personal Outcome Measures, or POMS,  a tool that provides valid and reliable measurement of individual quality of life.

Paying for Services From Medicaid to personal allowance, services at People Inc. are paid for in a variety of ways. The process can be confusing, but we are here to help. Your Medicaid service coordinator will work closely with you to ensure your loved one is receiving the benefits and services they need so they receive the best possible care. Please note, if your loved one resides in one of our residential sites, additional financial information will be provided to you. WHAT YOU NEED TO KNOW • The majority of services are paid for by your Medicaid coverage • You are responsible for maintaining your Medicaid coverage, but we are here to help you obtain or maintain this benefit • If you do not apply for coverage or do not complete the application process, then you must pay for the full cost of any services provided • All service costs will be disclosed to you upon enrollment BEFORE SERVICES CAN BEGIN Before your services can begin at People Inc., you must disclose certain benefit and financial information to us. Your service coordinator will tell you what information is needed. If your benefit or financial information changes during the time services are provided, you must notify us. Paying for Services (Continued) OTHER WAYS SERVICES CAN BE PAID In addition to Medicaid, services can be paid through: • Waiver funding, like the Home and Community Based Services (HCBS) Waiver • Supplemental Security Income • Personal allowance • Private pay Your service coordinator will work closely with you to identify all the ways your services can be paid for and help you apply for benefits. WHO IS RESPONSIBLE FOR PAYMENT In most cases, the person responsible for payment, called the payee, will be the individual him or herself, a designated family member or the servicing agency. The responsibilities of the payee include overall financial management, reporting any of the changes listed above to a service coordinator and paying any Social Security over payments. Privacy and Records Protecting the privacy of those we serve is very important to us. Your rights and responsibilities are reviewed upon admission into each program. At People Inc., we adhere to Jonathan’s Law, a regulation requiring that parents and legal guardians of children and adults receiving services are notified of incidents involving their loved ones. Jonathan’s Law allows qualified persons to access certain documents pertaining to incidents. People Inc. also complies with the Health Insurance Portability and Accountability Act (HIPAA) regulations. HIPAA laws are designed to provide privacy standards to protect patients’ medical records and other health information provided to health plans, doctors, hospitals and other health care providers. Developed by the Department of Health and Human Services, these standards provide patients with access to their medical records and more control over how their personal health information is used and disclosed.
