Weld Failure What happened: At C412 (Bond St.), the operator of a Hitachi 6ton Excavator noticed a crack on the dipper arm, close to where it connects to the quick hitch device. Further examination was carried out on the two remaining excavators, identifying hairline fractures to the welds in the Dipper end of the machine. This is the second type of incident of this nature within an 8-week period, although, the machine is being used for normal activities within its specified capabilities. Future Considerations: 

The duration for which the machinery is operated in a challenging environment should be monitored and managed.



Consider use of breaker attachment with Quick-Hitch.



Consider rotation in breaker attachment.



Pre-work checks on plant/machinery must always take place.
