FSL, also known as Foundation for Senior Living, has been caring for Arizonans of all ages since 1974. Our mission is to provide home and community-based services and develop energy-efficient, affordable housing to promote health, independence and dignity for all.

It begins with a compassionate heart. Factors that enhance successful aging include physical wellness, financial security, interactions with family and friends, and a sense of purpose. Successful aging begins with successful service. We believe quality care can be coordinated in one place to offer a safe home for over 1,000 Arizonans each night, nutritious meals for over 1,000 Arizonans each day, nearly 350,000 hours of respite or a break for caregivers each year, and over 28,000 hours of education for our clients, patients and caregivers. I joined the FSL team in January of 2015 as the new President and CEO, and I feel incredibly blessed to have been chosen to lead this wonderful organization. However, we also lost over 60 years of leadership and knowledge with the retirements of longtime President and CEO, Guy Mikkelsen, and Vice President, Linda Martin. The Board of Directors and I cannot thank them enough for their lifetime of service to this organization. We hope we have made them proud as we carry on their legacy.

Because of you, we achieved some exciting milestones in 2015.





FSL Real Estate Services received two Arizona Department of Housing Brian Mickelsen Housing Hero Awards, including: Outstanding Affordable Housing Initiative for 29 Palms in Phoenix, which cohouses seniors with adults on the autism spectrum and Exemplary Rural Multifamily Project for Pineview Manor in Payson. FSL’s Adult Day Health Services program served 531 clients, providing 320,712 hours of care, 197,682 miles of transportation and 38,455 nutritious meals to adults of all ages requiring assistance during the day.



We contracted with a public relations firm to raise awareness of FSL’s incredible programs and with a grant writer to increase funding in support of new and expanding initiatives.



We restructured the Executive Leadership team, resulting in a savings of $750,000 without a change or interruption to service delivery.

We could not do it without our many partners in the faith, healthcare, government, corporate, and philanthropic communities. I invite you to view our 2015 accomplishments. We focus on four areas of essential need:

Financials

Safe Home Provide a SAFE HOME for 1,044 Arizonans each night

Safe Home: We develop both single- and multi-family homes and we manage multi-family affordable housing complexes and residential behavioral health group homes throughout the state of Arizona. We also train and monitor sponsors of the Adult Foster Care and Adult Behavioral Health Therapeutic Home programs. Nutritious Meals: We provide 375,000 pounds of food to families in need from our two food pantries in the rural towns of Wickenburg and Williams. Additionally, we serve nearly 800 congregate and home-delivered meals each day. Health & Wellness: We offer services to empower community-based living, increase independence and enhance and maintain the highest functional quality of life possible for our clients. Some of these programs include: Medicare-certified, state-licensed skilled Home Health services, including support for ventilator- and trach-dependent patients; and non-medical Home Care to assist with personal care, housekeeping, meal preparation and respite for family caregivers. Education: Throughout all of our programs, we educate FSL patients and clients, family and professional caregivers, as well as members of the community, to enable them to maintain independence for themselves, their loved ones and their clients. We have adopted a new tagline: FAITH, SERVICE, LIFE. We are a FAITH-based charity that provides a wide range of SERVICEs to help people age in place throughout their LIFE.

We secured grants to help with Board recruitment and governance as well as strategic planning.

VICE CHAIR Lupe Solis Retired: AARP

FSL Home Improvements completed accessibility modifications for 534 aging or disabled homeowners. FSL’s Human Resources department completed its transition to a paperless employee filing system.

What You Can Do To Support Us:

FSL’s first fundraising appeal in more than 10 years was mailed to nearly 2,000 homes in spring 2015. FSL Home Energy Solutions reported 6,643 energy audits to the Environmental Protections Agency through the Arizona Home Performance with ENERGY STAR® program, completing 2,585 jobs. FSL’s Assisted Group Living program provided 29,249 bed days to 127 individuals with a behavioral health diagnosis. FSL’s SouthWest Building Science Training Center provided 2,376 hours of education to 139 students. FSL’s Adult Therapeutic Foster Program’s network of foster families provided 3,794 bed days to 14 clients, assisting them in achieving their service plan goals. FSL Real Estate Services single-family program sold 35 homes through the City of Phoenix Neighborhood Stabilization Program, bringing the total since 2009, to 160 homes. FSL staff provided more than 10,000 baths, 500,000 meals, and 335,000 miles of transportation to help those you love live independently. FSL’s Certified Therapeutic Recreational Therapist, Alicia Gonzales, won the RISE Award from Mercy Maricopa Integrated Care, for her work in developing innovative approaches to working with individuals with mental illness.
